Τα προκλητά δυναμικά κατέχουν σημαντική θέση στον προσδιορισμό των ουδών της ακοής σε παιδιά πολύ μικρά για να εξεταστούν με τις συμβατικές συμπεριφορικές μεθόδους και σε άλλους πληθυσμούς που παρουσιάζουν δυσκολίες κατά την εξέταση. Το ABR (Auditory Brainstem Response), μέθοδος που χρησιμοποιείται κατά τα τελευταία 50 χρόνια, έχει σήμερα ευρέως αναγνωριστεί ως η πιο αποτελεσματική τεχνική προκλητών δυναμικών και χρησιμοποιείται καθημερινά, με click και tone burst ως ερεθίσματα, για την εκτίμηση της ακουστικής ικανότητας σε βρέφη και παιδιά πολύ μικρά για άλλες εξεταστικές μεθόδους. The automatic response detection system used in ASSR recordings eliminates the need for subjective visual inspection of responses. The stimulus used in this study to evoke the ASSR was the narrowband CE-chirp, a kind of chirp that is a relatively very recent newcomer in commercially available equipments. This study aims at comparing and finding correlations between these two methods of electrophysiologic testing, in a large cohort of pediatric patients with various degrees of sensorineural hearing loss, examined by means of a sedated electrophysiologic testing at the Audiology Center in our Clinic during the period 2010-14. A total number of one hundred and thirty children, 90 males and 40 females (n = 260 ears) were finally seen, ranging in age from 5 to 79 months (mean age = 32.7 ± 14.0 months), with a variety of disabilities and disorders. The final statistical analysis for the comparison of these two methods has included on the one hand the threshold values obtained from click ABR and, on the other hand, the mean threshold values obtained from chirp ASSR from frequencies 1, 2 and 4 kHz as well as 2 and 4 kHz. The results of this study indicated strong correlations between click ABR and chirp ASSR thresholds averaged at 1000, 2000 and 4000 Hz (rs = .826, p < .001) as well as at 2000 και 4000 Hz (rs = .824, p < .001), and generally agree with the results of similar studies in the literature. In conclusion, it seems that ASSR is necessary for the exact quantification of residual hearing in the severe-to-profound hearing loss range, and at levels above the output limits of most ABR equipment. The “audiologic information” obtained through ASSR testing, particularly in the aforementioned range, is invaluable in the decision-making process for further intervention in children with sensorineural hearing loss (amplification, cochlear implantation), especially those children that cannot be measured with behavioral methods. These results support the use of ASSR as an alternative to ABR in the evaluation and management of childhood hearing loss.

Evoked potentials are critical in determining audiologic thresholds in children too young for traditional behavioral methods and in other difficult-to-test populations.
