Τα μη στεροειδή αντιφλεγμονώδη φάρμακα (NSAIDs) είναι η πιο ευρέως χρησιμοποιούμενη κατηγορία φαρμάκων για τη θεραπεία της φλεγμονής και γενικώς από τα συχνότερα συνταγογραφούμενα φάρμακα. Παρ’όλα αυτά παρουσιάζουν σημαντικές παρενέργειες που μπορεί να οδηγήσουν ακόμη και στο θάνατο, όπως αλλοιώσεις του γαστρεντερικού (ΓΕΣ) βλεννογόνου, αύξηση του χρόνου που απαιτείται για την πήξη του αίματος και νεφρική ανεπάρκεια. Ως μία πρόσφατη και πολλά υποσχόμενη προσέγγιση του προβλήματος παρουσιάστηκε αυτή της χρήσης αναστολέων κυκλοοξυγονάσης (COX)-2 οι οποίοι θεωρήθηκε ότι θα ήταν αποτελεσματικοί παράγοντες στη θεραπεία φλεγμονωδών παθήσεων επιδρώντας μόνο στην ισομορφή COX-2 η οποία είναι υπεύθυνη για τη δημιουργία φλεγμονής, χωρίς να επηρεάζουν την ισομορφή COX-1 η οποία είναι υπεύθυνη για την παραγωγή γαστροπροστατευτικών προσταγλανδινών στον γαστρεντερικό σωλήνα. Αυτά τα φάρμακα κυκλοφορούν ήδη και θεωρούνται ασφαλή όσον αφορά την επίδραση τους στο ΓΕΣ και δραστικά αντιφλεγμονώδη, όμως πρόσφατες μελέτες έδειξαν ότι έχουν κάποιες σημαντικές παρενέργειες. Μια άλλη προσέγγιση στην προσπάθεια μείωσης της τοξικότητας των NSAIDs είναι η προσθήκη μιας ομάδας που απελευθερώνει μονοξείδιο του αζώτου (NO) σ’αυτά τα μόρια. Κατ’αυτόν τον τρόπο έχουμε μια καινούρια κατηγορία αντιφλεγμονωδών, τα NO-NSAIDs. Το ΝΟ ενισχύει αρκετούς μηχανισμούς προστασίας του ΓΕΣ και μπορεί να μειώσει τα επίπεδα της φλεγμονής που οφείλεται σε οξειδωτικό stress. Τέλος είναι γνωστό ότι οι ελεύθερες ρίζες οξυγόνου συμμετέχουν στη δημιουργία φλεγμονωδών καταστάσεων. Στην παρούσα εργασία αναφέρουμε τη σύνθεση 26 ενώσεων, 13 από τις οποίες είναι καινούριες. Σχεδιάσαμε και συνθέσαμε 7 καινούρια παράγωγα του τολφαιναμικού οξέος (Τ.Ο.) τα οποία έχουν ομάδα δότη ΝΟ (ενώσεις 6-12) και 6 καινούρια παράγωγα της 2,6-δι-tert-βουτυλοφαινόλης (ενώσεις 20, 22-26) τα οποία είναι αμίνες και αμίδια της θειομορφολίνης, της μορφολίνης και της προλίνης. Another approach to reduce the toxicity of NSAIDs is the linking of a nitric monoxide (NO) releasing moiety to these compounds, thus, a new category of anti-inflammatory agents is emerging, the NO-NSAIDs. Nitric oxide is a small diatomic radical which plays an important physiological role in nervous, cardiovascular and immune systems. Its generation is controlled by the three isoforms of NO synthases (NOS), neuronal and endothelial NOS (nNOS and eNOS), which are constitutive and produce nanomolar amounts of NO important for normal cell function and tissue protection, and inducible NOS (iNOS) which generates high amounts of NO responsible for the destruction of invading pathogens as part of an overall inflammatory response. The endogenous tissue NO, generated constitutively by GI eNOS and nNOS, appears to play a key role in the chronic maintenance of gastrointestinal tissue integrity and in adaptive cytoprotection to injurious stimuli, perhaps acting synergistically with other cytoprotective prostaglandins. NO promotes several gastric defense mechanisms. In addition, NO may reduce inflammation connected to oxidative stress by scavenging reactive oxygen species which can adversely increase mucosal permeability and kill cells. Furthermore, it is found that NO and NO-derived reactive nitrogen species interact with peroxidases and lipoxygenases, altering the generation of prostaglandins and leukotrienes, which are signaling molecules involved in inflammation.
