Η νόσος του Parkinson είναι η δεύτερη πιο συχνή νευρολοεκφυλιστική ασθένεια προσβάλλοντας περίπου 1-2% του πληθυσμού άνω των 65 ετών. Αν και η αιτιολογία της νόσου είναι ακόμα άγνωστη το γενετικό υπόβαθρο της νόσου έχει καταγραφεί. Σκοπός της παρούσης εργασίας ήταν να διερευνήσει περαιτέρω το γενετικό υπόβαθρο της σποραδικής νόσου του Parkinson, μελετώντας γενετικούς πολυμορφισμούς/μεταλλάξεις στα γονίδια tau, GSK3beta και LRRK2. Προκειμένου να διερευνηθεί αν οι πολυμορφισμοί στο γονίδιο tau και/ή GSK3beta ενισχύουν την πιθανότητα εκδήλωσης της νόσου πραγματοποιήθηκε μελέτη συσχέτισης στον Ελληνικό πληθυσμό σε δείγμα 196 ασθενών και 163 υγιών ατόμων. Στην παρούσα μελέτη, ο απλότυπος MAPT H1 αντιπροσωπεύεται από ένα έλλειμμα 238bp στο ιντρόνιο 9 του γονιδίου tau το οποίο προσδιορίστηκε με τη μέθοδο PCR. Η σύγκριση της κατανομής συχνοτήτων των γονοτύπων και των απλοτύπων στις δύο ομάδες έγινε με τη δοκιμασία χ², ενώ για την ανάλυση απλοτύπων χρησιμοποιήθηκαν τα προγράμματα Haplo.score και PHASE ver.2.1. Στην παρούσα μελέτη βρέθηκε ότι ο απλότυπος Η1 υπεραντιπροσωπεύεται στους ασθενείς με Parkinson σε σχέση με την υγιή ομάδα ελέγχου (Ρ=0,003). Ο υποαπλότυπος G-A των πολυμορφισμών rs242562 και rs2435207 συσχετίστηκε επίσης με την νόσο (P=0,017). Καμία συσχέτιση δε βρέθηκε μεταξύ του πολυμορφισμού rs6438552 GSK3beta και της νόσου του Parkinson, ενώ ο γονότυπος C/C του πολυμορφισμού rs334558 GSK3beta βρέθηκε να έχει προστατευτικό ρόλο (Ρ=0,0005). Από την ανάλυση απλοτύπων προέκυψε ότι ο απλότυπος T-T των GSK3beta πολυμορφισμών υπεραντιπροσωπεύεται στους ασθενείς με Parkinson σε σχέση με τα υγιή άτομα (P=0,001) και η συσχέτιση αυτή ήταν ανεξάρτητη του απλοτύπου MAPT H1. Η μετάλλαξη G2019S LRRK2 δε βρέθηκε στην παρούσα μελέτη. Η πρωτεΐνη tau φαίνεται να εμπλέκεται στην παθογένεση πολλών νευροεκφυλιστικών ασθενειών, συμπεριλαμβανομένης και της νόσου του Parkinson. Στην παρούσα μελέτη, ο απλότυπος tau H1 βρέθηκε να έχει ισχυρή συσχέτιση με τη νόσο του Parkinson. Αξιοσημείωτο είναι το γεγονός ότι ο απλότυπος tau H1 έχει συσχετισθεί με αυξημένη έκφραση της πρωτεΐνης tau. Η αυξημένη έκφραση και η συσσώρευση της πρωτεΐνης tau πιστεύεται ότι είναι παθογενετική και νευροτοξική. Κινάσες tau, όπως η GSK3beta, φαίνεται να έχουν σημαντικό ρόλο στην νευροτοξικότητα της tau, καθώς η φωσφορυλίωση της tau οδηγεί στη συσσώρευσή της. Στην παρούσα μελέτη ο απλότυπος T-T των GSK3beta πολυμορφισμών υπεραντιπροσωπεύεται στους ασθενείς με Parkinson, υπογραμμίζοντας τον πιθανό γενετικό ρόλο του GSK3beta στην σποραδική νόσου του Parkinson. Όσον αφορά, την μετάλλαξη G2019S LRRK2, δεν είναι κοινή στον Ελληνικό πληθυσμό, ενισχύοντας την ανάγκη να εξεταστούν διαφορετικοί πληθυσμοί ώστε να εκτιμηθεί η συχνότητα της μετάλλαξης αυτής παγκοσμίως.

To determine whether polymorphisms in the tau and/or GSK3beta genes underpin susceptibility to Parkinson's disease (PD), we conducted a case-control association study in a Greek cohort of 196 PD cases and 163 healthy controls. Comparison of genotype and haplotype frequency distributions, in patients and controls, was done with the χ² test, whereas haplotype analysis was carried out with the use of Haplo.score and PHASE ver.2.1 programmes. In our study, the MAPT H1 haplotype was found to be significantly associated with PD (Ρ=0,003). The subhaplotype G-A of tau rs242562 και rs2435207 was also overrepresented in PD patients compared to controls (P=0,017). No association was detected between the rs6438552 GSK3beta polymorphism and PD, whereas the C/C genotype of the rs334558 GSK3beta polymorphism was found to exert a protective role (Ρ=0,0005). Haplotype analysis revealed that, the T-T haplotype of both GSK3beta polymorphisms was over-represented in PD patients compared to controls (P=0,001), and this association was independent of MAPT H1 haplotype. G2019S Lrrk2 mutation was not found in our study. Tau seems to be implicated in the pathogenesis of many neurodegenerative diseases, including Parkinson’s disease. In the present study, the tau H1 haplotype was significantly associated with PD. Importantly, the tau H1 haplotype has been associated with increased tau expression. Increased expression and accumulation of tau is believed to be pathogenic and neurotoxic. Tau kinases, such as GSK3beta, seem to have an important role in tau neurotoxicity, as phosphorylation of tau results to its accumulation. In the present study, the T-T haplotype of GSK3beta polymorphisms was overrepresented in PD patients, highlighting a potential genetic role of GSK3beta in sporadic PD. As far as the G2019S LRRK2 mutation is concerned it is not common in the Greek population, enhancing the need to screen different populations in order to estimate the frequency of this mutation globally.
