Εισαγωγή, σκοπός: Υπάρχουν ενδείξεις ότι σε ασθενείς με κίρρωση, αιμοδυναμικές παράμετροι, όπως η ΑΠ και η ΚΣ σχετίζονται με τη βαρύτητα της ηπατικής ανεπάρκειας. Σκοπός της μελέτης είναι η διερεύνηση της συσχέτισης μεταξύ 24ωρης καταγραφής της ΑΠ και της ΚΣ και κλινικών και βιοχημικών δεικτών της ηπατικής λειτουργίας. Ασθενείς και μέθοδοι: Ασθενείς με κίρρωση του ήπατος κάθε αιτιολογίας, άνω των 18 ετών, με ηπατική νόσο σταδίου Α ή Β κατά Child-Pugh υποβλήθηκαν σε μετρήσεις της ΑΠ και της ΚΣ στο ιατρείο με υδραργυρικό πιεσόμετρο και στηθοσκοπική τεχνική, σε μη επεμβατική 24ωρη καταγραφή της ΑΠ και της ΚΣ και σε εργαστηριακές εξετάσεις αίματος και ούρων. Αποτελέσματα: 51 ασθενείς ολοκλήρωσαν τη μελέτη (32 άνδρες), μέσης ηλικίας 57,4 έτη (±SD 11,3 έτη). Η βαθμολογία κατά Child-Pugh ήταν κατά μέσο όρο 6,7 στο σύνολο των ασθενών, 5,3 στους ασθενείς σταδίου Α και 8,2 στους ασθενείς σταδίου BC. Μεταξύ των ασθενών σταδίου Α και BC η μόνη στατιστικά σημαντική διαφορά στις παραμέτρους της 24ωρης καταγραφής ήταν στη διαστολική πίεση κατά τη νυκτερινή κατάκλιση (66,8 mmHg έναντι 61,8 mmHg αντίστοιχα, p<0,05). Οι μετρήσεις της ΑΠ στο ιατρείο διέφεραν στατιστικά σημαντικά ως προς τη διαστολική πίεση: 81,9 mmHg έναντι 73,8 mmHg αντίστοιχα στην πρώτη επίσκεψη (p<0,01) και 79,8 mmHg έναντι 72,5 mmHg αντίστοιχα στη δεύτερη (p=0,01). Μελετήθηκαν οι συσχετίσεις μεταξύ 24ωρης ΑΠ, INR, λευκωματίνης, χολερυθρίνης, ρενίνης και αλδοστερόνης ορού. Η διαστολική πίεση 24ώρου, ημέρας και νύκτας συσχετιζόταν θετικά με τη λευκωματίνη (r=0,33, p<0,05, r=0,38, p<0,01 και r=0,32, p<0,05 αντίστοιχα) και η συστολική πίεση κατά την ημέρα αρνητικά με τη ρενίνη (r=-0,30, p<0,05). Οι μετρήσεις της ΑΠ στο ιατρείο ανέδειξαν ισχυρότερες συσχετίσεις, συγκριτικά με την 24ωρη καταγραφή, αρνητικές με τη βαθμολογία κατά Child-Pugh, τη χολερυθρίνη, το INR, τη ρενίνη και την αλδοστερόνη και θετικές με τη λευκωματίνη. Οι μετρήσεις της ΚΣ 24ώρου, ημέρας και νύκτας ανέδειξαν ισχυρότερες συσχετίσεις με τις εργαστηριακές παραμέτρους, συγκριτικά με τις μετρήσεις της 24ωρης ΑΠ: θετικές με τη χολερυθρίνη (r=0,35, p<0,01, r=0,28, p<0,05, r=0,40, p<0,01 αντίστοιχα), τη βαθμολογία κατά Child-Pugh (r=0,32, p<0,05, r=0,24, NS, r=0,35, p<0,05), το INR (r=0,36, p<0,01, r=0,31, p<0,05, r=0,33, p<0,01), τη ρενίνη (r=0,49, p<0,001, r=0,48, p<0,001, r=0,43, p<0,01) και την αλδοστερόνη (r=0,44, p<0,001, r=0,40, p<0,01, r=0,44, p<0,001) και αρνητικές με τη λευκωματίνη (r=-0,36, p<0,01, r=-0,25, NS, r=-0,45, p<0,001). Οι μετρήσεις της ΚΣ στο ιατρείο έδειξαν παρόμοιες συσχετίσεις με αυτές της 24ωρης ΚΣ. Συμπεράσματα: Η υπόθεση ότι η αξιολόγηση της ΑΠ με 24ωρη καταγραφή είναι αξιόπιστος δείκτης ηπατικής ανεπάρκειας δεν επιβεβαιώθηκε από τη μελέτη αυτή. Αντίθετα η αύξηση της ΚΣ φάνηκε να συσχετίζεται καλύτερα με τους βιοχημικούς και ορμονικούς δείκτες της ηπατικής λειτουργίας. Επιπλέον η 24ωρη καταγραφή των παραμέτρων αυτών δεν φαίνεται να παρέχει περισσότερες πληροφορίες από τις συμβατικές μετρήσεις στο ιατρείο.

This study was conducted in order to assess the correlation between ambulatory blood pressure and heart rate measurement and established clinical and biochemical markers of liver function. Subjects and methods: Patients with liver cirrhosis, irrespective of etiology, sex and age (>18 years), with Child-Pugh stage of disease A or B, underwent office blood pressure and heart rate measurements with mercury sphygmomanometer, using the stethoscopic technique, non-invasive ambulatory blood pressure and heart rate monitoring for 24 hours and blood and urine laboratory tests. Results: 51 patients completed the study, 32 men, mean age 57,4 years (±SD 11,3 years). The cause of cirrhosis in 23 patients was alcohol abuse, in 20 viral hepatitis and in 8 else. 26 patients had Child-Pugh stage A disease and 23 stage B. 2 additional patients with stage C cirrhosis were included in the study and were analyzed as one group with stage B patients. Mean Child score was 6,7 in all patients, 5,3 in Child A group and 8,2 in Child BC group. Office BP measurements differed significantly between group A and group BC patients in diastolic pressures: 81,9 mmHg vs 73,8 mmHg in the 1st visit (p<0,001) and 79,8 mmHg vs 72,5 mmHg in the 2nd visit (p=0,01). The only statistically significant difference between the two groups in ambulatory blood pressure parameters was in night-time diastolic pressure, which was lower in group BC (66,8 mmHg in group A vs 61,8 mmHg in group BC, p<0,05). Statistical correlations between ambulatory blood pressure and heart rate parameters and INR, serum albumin, total bilirubin, renin and aldosterone concentrations were studied. Ambulatory diastolic blood pressure (overall, daytime and night-time) correlated positively with serum albumin (r=0,33, p<0,05, r=0,38, p<0,01 and r=0,32, p<0,05, respectively) and daytime systolic pressure correlated negatively with renin (r=-0,30, p<0,05). Office blood pressure measurements, compared to ambulatory ones, had stronger correlations: negative with Child score, total bilirubin, INR, renin and aldosterone and positive with albumin. Ambulatory heart rate measurements (overall, daytime, night-time), compared to ambulatory blood pressure measurements, had stronger correlations with laboratory parameters: positive with Child score: r=0,32, p<0,05, r=0,24, NS, r=0,35, p<0,05, total serum bilirubin: r=0,35, p<0,01, r=0,28, p<0,05, r=0,40, p<0,01, INR: r=0,36, p<0,01, r=0,31, p<0,05, r=0,33, p<0,01, renin: r=0,49, p<0,001, r=0,48, p<0,001, r=0,43, p<0,01, aldosterone: r=0,44, p<0,001, r=0,40, p<0,01, r=0,44, p<0,001 and negative with serum albumin: r=-0,36, p<0,01, r=-0,35, NS, r=-0,45, p<0,001. Office heart rate correlations were similar to ambulatory ones. Conclusions: The hypothesis that assessment of blood pressure with ambulatory measurement in patients with liver cirrhosis will be proved a reliable marker of hepatocellular failure was not verified in this study. On the contrary, elevation of heart rate seems to correlate better with biochemical and hormonal markers of liver function. Moreover, the evaluation of blood pressure and heart rate with ambulatory measurement does not seem to offer more information than conventional office measurements.
