Ο TF (tissue factor) είναι ο βασικός in vivo πυροδοτητής της πήξης ο οποίος συμμετέχει σε διάφορα μοντέλα φλεγμονής. Τα σύνδρομα ARDS (acute respiratory distress syndrome) στους ενήλικες και RDS (respiratory distress syndrome) στα νεογνά έχουν κοινές παθοφυσιολογικές εκδηλώσεις, όπως είναι η συσσώρευση φλεγμονωδών μεσολαβητών στον μικροκυψελιδικό χώρο, η διαπήδηση των ουδετεροφίλων σε αυτόν, η συσχέτηση της βαρύτητας του συνδρόμου από τον αριθμό τους και η δημιουργία μεμβρανών υαλίνης στον κυψελιδικό χώρο που προκύπτουν από την εναπόθεση ινικής, το τελικό παράγωγο του πηκτικού μηχανισμού. Παράλληλα, μελέτες κατάδειξαν ότι η θρομβίνη, μια πρωτεΐνη της κοινής οδού της πήξης, μπορεί να επάγει την έκφραση του CTGF (connective tussue growth factor) ο οποίος είναι ένα κατεξοχήν προϊνωτικό μόριο. Ξεκινώντας από το προθρομβωτικό μικροκυψελιδικό περιβάλλον αυτών των συνδρόμων και από το γεγονός ότι τα ουδετερόφιλα μπορούν να εκφράσουν αλλά και να παράγουν λειτουργικό ιστικό παράγοντα, εξετάστηκε η ικανότητα των ουδετεροφίλων να εκφράζουν TF στον κυψελιδικό χώρο κατά την εξέλιξη αυτών των συνδρόμων. Επίσης, διερευνήθηκε η σχέση της έκφρασης TF και CTGF στα νεογνά με RDS που εξελίχθηκαν σε BPD. Παρατηρήθηκε ότι τα κυψελιδικά ουδετερόφιλα των ασθενών με ARDS παράγουν περισσότερο TF και asTF από τα ουδετερόφιλα της περιφέρειάς τους και συντελούν με αυτό τον τρόπο στη δημιουργία του θρομβωτικού περιβάλλοντος στους πνεύμονες των ασθενών. Η θρομβωτική ιδιότητα των ουδετεροφίλων παρατηρήθηκε μόνο τοπικά στον κυψελιδικό χώρο και όχι στην περιφέρεια των ασθενών. Παράλληλα, παρατηρήθηκε υπερέκφραση TF και CTGF στα νεογνά που εκδήλωσαν BPD (bronchopulmonary dysplasia) σε σχεση με τα νεογνά με RDS. Η κινητική ανάλυση στα νεογνά αυτά έδειξε παράλληλη μεταβολή του TF και CTGF με την πάροδο του χρόνου. Οι in vitro διεγέρσεις υγιών ουδετεροφίλων με BALF (bronchoalveolar lavage fluid) νεογνών έδειξαν μια εξαρτώμενη από τη θρομβίνη και τον TF επαγωγή του CTGF σε επίπεδο RNA στα κύτταρα αυτά. Τα αποτελέσματα είναι μία ακόμη απόδειξη της σύνδεσης της φλεγμονής με τη θρόμβωση σε in vivo μοντέλα με βασικό πρωταγωνιστή τον ιστικό παράγοντα, αλλά και της ικανότητας των ουδετεροφίλων να εκφράζουν λειτουργικό TF. Προτείνουμε λοιπόν ένα πιθανό μηχανισμό μέσω του οποίου αυτά τα φλεγμονώδη κύτταρα συντελούν στη βαρύτητα των δύο αυτών συνδρόμων. Επιπρόσθετα, στην περίπτωση τον νεογνών με BPD φαίνεται μία συμμεταβολή της έκφρασης του TF και του CTGF, που δηλώνει μία πιθανή συσχέτιση της θρόμβωσης με την ίνωση.

Moreover, we examined the correlation between the expression of TF and CTGF in infants with RDS that developed BPD (bronchopulmonary dysplasia). Alveolar neutrophils of ARDS patients expressed more TF and asTF compared to their peripheral blood neutrophils and thus they conrtribute to the prothrombotic environment of the patients’ lungs. This prothrombotic activity was localised in the microalveolar environment and was not observed in the peripheral blood of the patients. Furthermore, we observed TF and CTGF overexpression in infants that developed BPD compared to RDS ones. Kinetic analysis in these infants illustrated the same pattern of expression of the two genes during the course of the syndrome. In vivo stimulations of heathy neutrophils with BALF from infants demonstrated in RNA level a thrombin and TF-dependent induction of CTGF expression. These findings are another evidence of the crosstalk between inflammation and coagulation in in vivo models, in which TF plays the central role. Also, this study is another verification of the ability of neutrophils to express and produce functional TF. In this study it is suggested a possible mechanism through which these inflammatory cells contribute to the severity of these syndromes. In addition, in BPD infants a correlation between TF and CTGF expression was observed, suggesting a possible correlation between coagulation and fibrosis.
