ΕΙΣΑΓΩΓΗ: Ο καρκίνος του τραχήλου της μήτρας αποτελεί ένα από τα σημαντικότερα προβλήματα της Δημόσιας Υγείας και μια από τις επικρατέστερες γυναικολογικές κακοήθειες. Η λοίμωξη ΗPV είναι το πιο συχνά σεξουαλικώς μεταδιδόμενο νόσημα και η αιτιολογική συσχέτιση της με τον καρκίνο του τραχήλου της μήτρας είναι αδιαμφισβήτητη. Ο σκοπός της έρευνας ήταν η εκτίμηση της εμβολιαστικής κάλυψης και ο προσδιορισμός του επιπέδου γνώσης για το εμβόλιο HPV φοιτητριών τριτοβάθμιας εκπαίδευσης. ΜΕΘΟΔΟΛΟΓΙΑ: Δείγμα της έρευνας αποτέλεσαν 3.153 φοιτήτριες τριτοβάθμιας Εκπαίδευσης ηλικίας 18-26 ετών. Ανώνυμο ερωτηματολόγιο χρησιμοποιήθηκε ως εργαλείο, το οποίο περιελάμβανε ερωτήσεις που αφορούσαν στα δημογραφικά χαρακτηριστικά όπως η ηλικία, εκπαιδευτικό επίπεδο των συμμετεχουσών και των γονέων τους, μόνιμη σχέση ή όχι, πρόσβαση στις υπηρεσίες υγείας, καθώς και μεταβλητές που αφορούν στην υγεία και στη σεξουαλική συμπεριφορά όπως συνήθειες καπνίσματος, επίσκεψη σε γυναικολόγο, προηγούμενη σεξουαλική εμπ ...

ΑΠΟΤΕΛΕΣΜΑΤΑ: Η εμβολιαστική κάλυψη ήταν 25,8 %. Η νεαρή ηλικία, η Πανεπιστημιακή εκπαίδευση, η επίσκεψη σε γυναικολόγο, η χρήση προφυλάξεων, η εύκολη πρόσβαση σε υπηρεσίες υγείας, αποτέλεσαν θετικούς παράγοντες για εμβολιασμό. Επιπρόσθετα, όσων οι γονείς είχαν υψηλότερο εκπαιδευτικό επίπεδο είχαν διπλάσια πιθανότητα στο να εμβολιαστούν. Η εμβολιαστική κάλυψη ήταν 27,3% και 24,9% για φοιτήτριες Επιστημών Υγείας και μη, αντίστοιχα. Ο εμβολιασμός σχετιζόταν θετικά με το υψηλό επίπεδο γνώσης, ενώ οι φοιτήτριες Επιστημών Υγείας είχαν υψηλότερα επίπεδα γνώσης για το εμβόλιο του ΗPV. Ωστόσο οι φοιτήτριες που είχαν υψηλά σκορ σε γενικούς αντιληπτούς φραγμούς για τον εμβολιασμό, στα μη αντιληπτά γενικά και ειδικά οφέλη, ήταν πιθανότερο να μην συμμορφωθούν με τον εμβολιασμό. ΣΥΜΠΕΡΑΣΜΑΤΑ: Η συμμόρφωση με τον εμβολιασμό ΗPV των φοιτητριών τριτοβάθμιας Εκπαίδευσης στην Ελλάδα, είναι σε μη αποδεκτά επίπεδα χαμηλή, παρά το γεγονός ότι ο εμβολιασμός είναι δωρεάν και το επίπεδο γνώσης και εμβολιαστικής κάλυψης του δείγματος της έρευνας, παρατηρήθηκε κάτω από τα επιθυμητά επίπεδα. Ακόμα και οι φοιτήτριες Επιστημών Υγείας είχαν χαμηλά επίπεδα γνώσεων σχετικά με τον εμβολιασμό ΗPV. Σε εθνικό επίπεδο είναι επιτακτική ανάγκη η δημιουργία Εθνικού Προγράμματος Πρόληψης. Μόνο όταν το ποσοστό εμβολιαστικής κάλυψης του γυναικείου πληθυσμού αγγίξει τα ποσοστά της Ευρώπης, θα υφίσταται πρόληψη του καρκίνου του τραχήλου της μήτρας στην Ελλάδα. ΠΡΟΤΑΣΕΙΣ: Απαιτούνται παρεμβάσεις Δημόσιας Υγείας για την εξασφάλιση πρόσβασης στο εμβόλιο σε όλους τους φοιτητές και η κάλυψή του από την κοινωνική ασφάλιση των Ιδρυμάτων, καθώς και πραγματοποίηση προγραμμάτων ενημέρωσης στα σχολεία, στα ιδρύματα, στα νοσοκομεία και στις υπηρεσίες της κοινότητας στοχεύοντας στην αύξηση των γνώσεων σχετικά με τον εμβολιασμό και κατ’ επέκταση σε αυξημένη εμβολιαστική κάλυψη.

Researchers have also evaluated individuals attitudes towards HPV and the HPV vaccination by utilizing the Health Belief Model (HBM). Methods:In 2010, a convenience sample of 3153 female higher education students aged between 18 and 26 years were surveyed. Data was collected through a self-completed questionnaire. The questionnaire included items of demographic characteristics such as age, educational level of participants and of their parents, place of residence, employment status, relationship status (“stable” or “not stable”), and accessibility to Health Care Units (“easy” or “difficult” access), as well as health and sexual behavior variables such as smoking habits, visits to a gynaecologist (“ever” versus “never”), previous sexual experiences, age of first sexual intercourse (≤17, >17), number of lifetime sexual partners (≤2, >2) and contraceptive behavior.Results: The number of women who were vaccinated was 25.8% (95% CI: 24.3–27.4). Students aged between 18 and 20 years, those who were University students, who had visited a gynaecologist, who always used male condoms, who did not smoke, who were not in a stable relationship, who did not work and those who had easy access to Health Care Services, had higher probability to administer the vaccine. Moreover, it is noteworthy that those whose parents had a higher education were almost twice as likely to report vaccination. Additionally, vaccination coverage was 27.3% and 24.9% for health sciences and non-health sciences students respectively (p < 0.01). Being vaccinated was positively and significantly associated with a high level of knowledge among the total sample (OR 1.64 95% CI 1.38–1.95), among non-health sciences students (OR 1.84 95% CI 1.48–2.28) and among health sciences students (OR 1.34 95% CI 1.01–1.78). Health sciences students had higher knowledge levels of the HPV vaccine than non-health sciences students (OR 1.83 95% CI 1.57–2.14). Likewise, the participants who had high scores in “general perceived barriers” (OR=1.67; 95%CI: 1.51-1.85) and in “perceived barriers to vaccination” (OR=1.66; 95%CI: 1.44-1.92) were more likely not to be vaccinated. Also, those who had high scores in “no perceived general benefits” (OR=1.97 95%CI: 1.66-2.34) and in “no perceived specific benefits” (OR=2.77 95%CI: 2.34-3.29) were more likely to report non-compliance with vaccination. Additionally, the use of male condoms was associated with age, educational level, occupational, relationship and smoking status, parents educational level, children, age of first sexual intercourse, use of male condom during first sexual intercourse and gynaecologist visits. The use of oral contraceptives (OCs) was associated with age, relationship status, number of sexual partners and gynaecologist visits. The use of emergency contraception (EC) was associated with age, occupational, relationship and smoking status, age of first sexual intercourse, use of male condom during first sexual intercourse, number of sexual partners and gynaecologist visits. Conclusions: Compliance with the HPV vaccine among female higher education students in Greece is unacceptably low despite the fact that vaccination is free-of-charge. Likewise, the level of knowledge and vaccine uptake among the study sample was observed below desirable levels. High knowledge is positively associated with vaccine uptake. It is significant that even the health sciences students had low levels of knowledge regarding to HPV vaccination. Also, it is worth mentioning that the utility of the Health Belief Model (HBM) constructs of perceived barriers and benefits in understanding intentions to be vaccinated. Participants who had negative perceptions about the vaccine in general (Lacked information about the HPV vaccine, felt the cost of vaccination was a barrier and had misconceptions about HPV vaccination) had a lower adherence to the HPV vaccine.
