Η ονδανσετρόνη είναι ένας ισχυρός και εξαιρετικά εκλεκτικός ανταγωνιστής των υποδοχέων σεροτονίνης (5-HT3), και χρησιμοποιείται για την αντιμετώπιση της ναυτίας και του εμέτου που προκαλούνται από την κυτταροτοξική χημειοθεραπεία και ακτινοθεραπεία. Η δράση της οφείλεται στον ανταγωνισμό των 5-HT3 υποδοχέων στους νευρώνες που βρίσκονται τόσο στο περιφερικό όσο και στο κεντρικό νευρικό σύστημα. Η ονδανσετρόνη χορηγείται με ενδομυική ή αργή ενδοφλέβια ένεση με την μορφή υδροχλωρικού άλατος, από το στόμα με την μορφή άλατος ή βάσης, ή από το ορθό με τη μορφή βάσης. Οι δόσεις εκφράζονται ως βάση. Για την αποτροπή της πρόκλησης εμέτου, μετά την εμετογόνο χημειοθεραπεία ή ακτινοθεραπεία, χορηγούνται 8 mg από το στόμα, δύο φορές την ημέρα μέχρι και πέντε ημέρες μετά το τέλος της θεραπείας. Η διαδερμική χορήγηση της ονδανσετρόνης, εάν ήταν δυνατόν να επιτευχθεί, θα παρουσίαζε σημαντικά πλεονεκτήματα σε σύγκριση με τις συμβατικές οδούς χορήγησης του φαρμάκου που χρησιμοποιούνται μέχρι σήμερα, δηλαδή την από του στόματος και την ενδοφλέβια οδό, όπως αποφυγή του φαινομένου πρώτης διόδου και επίτευξη σταθερών επιπέδων του φαρμάκου στο πλάσμα. Σκοπός της παρούσας μελέτης ήταν η αξιολόγηση της διαδερμικής διαπερατότητας της ονδανσετρόνης ως πρώτο βήμα για την ανάπτυξη διαδερμικών θεραπευτικών συστημάτων, από τα οποία το φάρμακο θα αποδεσμεύεται με τον επιθυμητό ρυθμό για την επίτευξη θεραπευτικής δράσης. Για τα in vitro πειράματα διαδερμικής διαπερατότητας χρησιμοποιήθηκαν τροποποιημένα κύτταρα διάχυσης του Franz κάθετου τύπου και ανθρώπινη επιδερμίδα από δέρμα διαφορετικών δοτών του ιδίου φύλου. Για τη διερεύνηση της επίδρασης του «είδους», της «συγκέντρωσης του επιταχυντή διαβατότητας» και του «δέρματος», χρησιμοποιήθηκε 8132 ασύμμετρος παραγοντικός σχεδιασμός. Οι συνταγές που περιείχαν λαουρικό ή ελαϊκό οξύ, ως επιταχυντές διαβατότητας, εμφάνισαν τις μεγαλύτερες τιμές Q (ποσότητα της ονδανσετρόνης (μg/cm2) που διαπερνούσε την επιδερμίδα ανά μονάδα επιφανείας) στις 24, 48 και 72 h από την έναρξη του πειράματος. Οι υπόλοιποι επιταχυντές διαβατότητας κατατάσσονται ανάλογα με την αποτελεσματικότητα τους, με την ακόλουθη σειρά: λαουρική αλκοόλη > Azone® > Myverol® 18-99 > η κινεόλη > ελαϊκή αλκοόλη > NMP. Επιπλέον, η συγκέντρωση του επιταχυντή διαβατότητας και το δέρμα επηρέαζαν στατιστικώς σημαντικά την απορρόφηση του φαρμάκου. Από τα αποτελέσματα που προέκυψαν, οι συνταγές που περιείχαν, ως επιταχυντές διαβατότητας, λαουρικό ή ελαϊκό οξύ σε συγκέντρωση 5% ή 10%, αύξαναν σημαντικά τον ρυθμό διαδερμικής διαπέρασης. Επομένως οι συνταγές αυτές θα μπορούσαν να χρησιμοποιηθούν για την ανάπτυξη διαδερμικών θεραπευτικών συστημάτων Ένας ακόμη στόχος της μελέτης ήταν η διερεύνηση του μηχανισμού δράσης του λαουρικού οξέος, ως επιταχυντή διαβατότητας, και η δημιουργία ιονικού ζεύγους μεταξύ του ονδανσετρόνης και του λιπαρού οξέος. Για το σκοπό αυτό χρησιμοποιήθηκαν διάφορες τεχνικές. Από τα πειράματα κατανομής παρατηρήθηκε αυξημένη λιποφιλία της ονδανσετρόνης σε τιμές pH που το φάρμακο βρίσκεται υπό την ιονισμένη του μορφή, πιθανώς ως αποτέλεσμα της δημιουργίας λιπόφιλου ιονικού ζεύγους μεταξύ του φαρμάκου και του επιταχυντή διαβατότητας.

In the present study, the in vitro permeation of ondansetron through human cadaver epidermis, as a preliminary step towards the development of a transdermal therapeutic system, was investigated. In vitro release studies were carried out using modified Franz diffusion cells and human epidermis, taken from cadaver skin by heat separation technique. In order to estimate the effect of the type and the concentration of the penetration enhancers and the skin from different donors, an 8132 asymmetrical factorial design was used. Formulations containing lauric acid and oleic acid as penetration enhancers, showed the largest Q values (amounts of ondansetron permeated per unit area of epidermal membrane (μg/cm2)) at 24, 48 and 72 h, as well as steady state flux values, among all formulations tested. The other enhancers increased the flux in the following order: lauryl alcohol > glycerol monooleate > Azone® > cineole > oleyl alcohol > 1-methyl-2-pyrrolidinone. Moreover, the concentration of the penetration enhancer and the type of the skin were proved to affect significantly the permeation rate of ondansetron through human epidermis. From the results obtained it was shown that the formulations containing lauric acid or oleic acid at 5% or 10% could increase sufficiently the permeation of ondansetron. Therefore the transdermal administration of ondansetron seems feasible. The formation of ion pair between ondansetron and lauric acid, as a possible mechanism of its enhancing action, was also investigated. Several techniques were used to reveal the formation of an ion pair complex. Partitioning experiments, where the n-octanol/water coefficient was measured, showed an increase in the distribution coefficient in the presence of the acid, possibly as a result of the formation of more lipophilic ion pairs between the charged molecules of ondansetron and lauric acid. Further evidence of complex formation between ondansetron and lauric acid, was gained from the 13C-NMR spectra of ondansetron, lauric acid and their mixture (molar ratio 1:1). The NMR spectra revealed alterations to the magnetic environment of the carbon atoms adjacent to the ionized group, which are the carbonyl group of the acid and the nitrogen of the imidazole ring of ondansetron. This evidence substantiates the theory of ion pair formation. Finally thermal analysis of the of the binary mixtures of ondansetron and lauric acid revealed the formation of an addition compound, with different melting point from pure ondansetron and lauric acid, which is thermodynamically favored. It is evident therefore that the enhancing effect of lauric acid on the transdermal permeation of ondansetron can be attributed not only on its direct effect on the skin structure, but also to the formation of an ion pair between the drug and the enhancer.
