Αντικείμενο της εργασίας ήταν η μελέτη της διατομικής μεταβλητότητας του vWAg, του vWF:RCof, του παράγοντα VIII:C, της κλίσης και του λόγου vWAg/VÏII:C σε υγιή παιδία ως και των παραγόντων που προσδιορίζουν την μεταβλητότητα αυτή. Η νόσος von Willebrand αποτελεί η συχνότερη κληρονομική αιμορραγική διαταραχή (επιπολασμός: 0,8%-1,3%) (226,). Η νόσος von Willebrand οφείλεται σε ποσοτική (τύπος 1 και 3) ή ποιοτική (2, 2Α, 2Β, 2Ν, 2Μ) διαταραχή του παράγοντα vW. Η διάγνωση της νόσου λόγω της απουσίας παθογνωμονικών κλινικών ευρημάτων είναι εργαστηριακή και βασίζεται σε λειτουργικές και ανοσολογικές μεθόδους προσδιορισμού του παράγοντα vW. Μελετήθηκαν 100 παιδιά ηλικίας 1 έως 14 ετών (διάμεση τιμή 5,5 έτη) 58 αγόρια και 45 κορίτσια με σωματικό βάρος από 9 έως 50Kg (διάμεσο σωματικό βάρος 21Kg) και ύψος από 68 έως 159cm (διάμεση τιμή 118,5cm). Ο προσδιορισμός α) του vWAg έγινε με ανοσοηλεκτοφόρηση κατά Laurell, β) της vWF:RCof με πλυμένα και μονομοποιημένα αιμοπετάλια στο συγκολλητινόμετρο ΡΑΡ-4 και γ) του παράγοντα VIII:C με την μέθοδο του ΑΡΤΤ σ'ένα στάδιο στο πηξιόμετρο MLA-800. Ο προσδιορισμός των υπόλοιπων παραμέτρων έγινε με τις συνηθισμένες εργαστηριακές τεχνικές. Τα αποτελέσματα συνοψίζονται ως κάτωθι:α) οι φυσιολογικές τιμές vWAg στα παιδιά κυμαίνονται από 33 u/dl έως 204 u/dl με μέση τιμή 89u/dl±35u/dl,β) οι φυσιολογικές τιμές του vWF: RCof κυμαίνονται από 33 u/dl έως 250 u/dl, με μέση τιμή 94u/dl±40 u/dl,γ) οι φυσιολογικές τιμές της κλίσης κυμαίνονται από 28 έως 58 με μέση τιμή 33 ±8δ) οι φυσιολογικές τιμές του παράγοντα VIII:C κυμαίνονται από 31 u/dl έως 232 u/dl με μέση τιμή 120u/dl±48 u/dl, (Πίνακας 2).Από την Λεπτομερέστερη ανάλυση με πολυμεταβλητές μεθόδους προέκυψε ότι1. οι τιμές του vWAg και του παράγοντα VIII:C δεν εξαρτώνται από τις 17 ανεξάρτητες μεταβλητές που μελετήθηκαν2. τα επίπεδα του vWF:RCof σχετίζονται θετικά με το ανάστημα και τέλος3. η κλίση εξαρτάται από το ανάστημα και λιγότερο από την ηλικία.Παρά τις καλές συσχετίσεις μεταξύ του vWAg και του vWF:RCof (r= +0,404 με ρ < 0,001), του vWF:RCof και του παράγοντα VIII:C (r= +0,608 με ρ < 0,000) και του vWAg και του παράγοντα VIII:C (r=0,32, ρ=0,007) που διαπιστώθηκαν, δεν είναι δυνατόν να αντικατασταθούν οι καθέκαστα προσδιορισμοί από έναν και μόνον λόγω της φύσης των προσδιορισμών (λειτουργικοί ή ανοσολογικοί προσδιορισμοί).

Its definitive diagnosis requires documentation of threefactors: bleeding, low levels of qualitively normal vWF, and inheritance(350). In the absence of any of these factors the diagnosis may be onlymerely possible, or even unacceptable. Laboratory diagnosis of vWDincludes screening tests and confirmatory tests. The screening tests includesplateletes count, bleeding time (BT), prothrombin time (PT) and activatedpartial thromboplastin time (APTT). The confirmatory tests includeprocoagulant factor VIII:C, vWF activities (vWF:Rcof), von Willebrandfactor antigen (vWAg), Ristocetin Inducet Platelet Aggregation (RIPA),Botrocetin Inducet Platelet Aggregation (ΒΙΡΑ). Bleeding time weredetermined by Duke technique and platelets counts were made on EDTAanticoagulantblood using an electroninic counter H2 (Technicon). The PT,APTT and fibrinogen was measured with an MLA 800, coagulation timerand reactants from Dade. Activity of vWF was maesured by the ristocetincofactor assay with formalin-fixed platelets on the aggregometre PAP-4(Biodata). Factor VIII:C was maesured with a one-stage partialthromboplastin time technique with dilutions in immunoabsorbed factorVIII:C-deficient plasma (Dade). The vWAg was maesured by the Laurellelectroimmunoassay technique using a polyclonal antiserum anti-vWFantigen from Biopool. ABO blood groups and Lewis were determined usingstandard tube methods.The normal range of vWF in children was difined in 45 gils and 58boys aged 2 to 14 without a personal or family history of hemorrage. Thevalues were not normally distributed and showed significant positivekurtosis and skewness. Separate normal ranges were calculated for Ο andnon-O subjects. Subjects of group Ο had a significantly lower mean vWFvalue (75,9%) than subjects of group A (101%), Β (97%), or AB (124%) (ρ <0,001 regression test). The distribution of vWF was independent of sexand age. The strict correlation were found between vWF and vWAg (p <0,001) and between vWF and VIII:C (p < 0,05). No changes in vWF levelswere found in relation to the time of blood sampling.Among persons belonging to blood group O, those with the Leaantigen had a higher concentration of vWAg than individuals lacking Lea.Le (b-a+) people are nonsecretors and Le (b+a-) people are secretors ofABH substances. Thus, the lowest concentration of vWAg are found ingroup Ο secretors. The Lewis substances Lea and Leb are closely relatedto the ABH substances as both can be produced from the same precursorsubstance.An assay for vWF (ristocetin cofactor activity) was found to be thesingle most useful test for identification of carriers and symptomaticmembers in kindreds with the disease.
