Η ERCP θεωρείται το gold standard για τη διάγνωση και θεραπεία της χοληδοχολιθίασης. Εξαιτίας του επεμβατικού χαρακτήρα της εξέτασης συνοδεύεται συχνά από επιλοκές όπως η μετά- ERCP παγκρεατίτιδα, η χολαγγειίτιδα, η αιμορραγία και η διάτρηση. Πρέπει να εκτελείται μόνο όταν το όφελος για τον ασθενή είναι μεγαλύτερο των κινδύνων για επιπλοκή.Η μετά-ERCP παγκρεατίτιδα είναι η συχνότερη επιπλοκή της εξέτασης. Διάφοροι παράγοντες σχετικοί με τον ασθενή, την εξέταση και τον ενδοσκόπο έχουν σχετισθεί με αυξημένα ποσοστά της επιπλοκή. Στη μελέτη μας το γυναικείο φύλο, η μικρότερη ηλικία, το επίπεδο φύμα του Vater, ο δύσκολος καθετηριασμός και η αποτυχία καθετηριασμού του χοληδόχου πόρου σχετίστηκαν με στατιστικά σημαντικά αυξημενο ποσοστό μετά-ERCP παγκρεατίτιδας. Πρέπει να εκτελείται μόνο όταν το όφελος για τον ασθενή είναι μεγαλύτερο των κινδύνων για επιπλοκή.Η μετά-ERCP παγκρεατίτιδα είναι η συχνότερη επιπλοκή της εξέτασης. Διάφοροι παράγοντες σχετικοί με τον ασθενή, την εξέταση και τον ενδοσκόπο έχουν σχετισθεί με αυξημένα ποσοστά της επιπλοκή. Η συμμετοχή εκαιδευόμενου ιατρού δεν επηρεάζει το ποσοστό επιτυχίας και μετά-ERCP επιπλοκών, αλλά σχετίζεται με αυξημένο ποσοστό καταστροφικών επιπλοκών.Ο καλύτερος τρόπος αποφυγής των επιπλοκών είναι η σωστή επιλογή των ασθενών που θα υποβληθούν στην εξέταση. Η χοληδοχοθίαση είναι η συχνότερη ένδειξη για ERCP. Η MRCP σε μαγνητικό τομογράφο 3Τ ή 1.5Τ είναι η απεικονιστική εξέταση με τη μεγαλύτερη ακρίβεια (75.4%- 81.5%) στη διάγνωση της χοληδοχολιθίασης.Η κλινική εικόνα ανιούσας χολαγγειίτιδας σε συνδυασμό με διάταση του χοληδόχου πόρου από τον διακοιλιακό υπέρηχο σχετίζεται με παρουσία χοληδοχολιθίασης στο 85% των περιπτώσεων. Ο συνδυασμός των δύο παραμέτρων μας επιτρέπει τη διενέργεια ERCP χωρίς περαιτέρω απεικονιστικές εξετάσεις.Σε ασθενείς με χολολιθίαση και χοληδοχολιθίαση μετά από αποτυχία της προεγχειρητικής ERCP, ή σε ασθενείς υψηλού κινδύνου για μετά-ERCP παγκρεατίτιδα το LERV αποτελεί μία ασφαλή και αποτελεσματική εναλλακτική μολονότι η επανάληψη της κλασικής ERCP με αναισθησιολογική κάλυψη έχει μεγαλύτερο ποσοστό επιτυχίας.Η προφυλακτική χορήγηση υπόθετου δικλοφενάκης 50 mg 30 λεπτά προ της εξέτασης μειώνει το ποσοστό της μετά-ERCP παγκρεατίτιδας συγκριτικά με το εικονικό φάρμακο (3% vs 6.5%). Η διαφορά ήταν στατιστικά σημαντική. Η απουσία ανεπιθύμητων ενεργειών, ο εύκολος εφάπαξ τρόπος χορήγησης, και το χαμηλό κόστος του φαρμάκου, το καθιστούν ιδανικό στην πρόληψη της μετά-ERCP παγκρεατίτιδας, σε ασθενείς χαμηλού και υψηλού κινδύνου για μετά- ERCP παγκρεατίτιδα.Η προφυλακτική χορήγηση σωματοστατίνης 6mg σε στάγδην έγχυση 24h μειώνει το ποσοστό της μετά-ERCP παγκρεατίτιδας συγκριτικά με το εικονικό φάρμακο (3% vs 6.5%). Η διαφορά ήταν στατιστικά σημαντική. Η στάγδην έχυση για 24h και το κόστος του φαρμάκου, σε συνδυασμό με το χαμηλή επίπτωση της επιπλοκής που θέλουμε να προλάβουμε καθιστά την καθολική χορήγηση του φαρμάκου πιθανώς αναποτελεσματική. Είναι προτιμότερη η χρήση της σε ασθενείς υψηλού κινδύνου για μετά-ERCP παγκρεατίτιδα που θα είναι και το όφελος μεγαλύτερο.Η διενέργεια προσφιγκτηροτομής δεν αυξάνει το ποσοστό επιπλοκών και μετά-ERCP παγκρεατίτιδας.129Η προφυλακτική τοποθέτηση παγκρεατικής ενδοπρόθεσης σε ασθενείς που κατά τη διάρκεια της εξέτασης εμφανίζουν παράγοντες αυξημένου κινδύνου για μετά-ERCP παγκρεατίτιδα μειώνει το ποσοστό εμφάνισης και τη βαρύτητα της μετά-ERCP παγκρεατίτιδας, συγκριτικά με τους ασθενείς που λαμβάνουν μόνο υπόθετο δικλοφενάκης.Σε περιπτώσεις αδυναμίας πλήρους καθαρισμού του χοληδόχου πόρου είναι αναγκαία η προσθήκη ή τροποιήση της αντιβιοτικής αγωγής και για gram αρνητικά μικρόβια

The Endoscopic Retrograde Cholangiopancreatography (ERCP) is the classic examination, leading to the diagnosis and treatment of both benign and malignant biliary and pancreas disorders. The invasive character of this examination is associated with certain complications, such as pancreatitis, cholangitis, haemorrhage and perforation. The aim of this study is to identify the high risk factors that can lead to the emergence of post-ERCP complications, so that their severity can be either prevented or alleviated, by the means of an early preventional pharmacological treatment and precautionary techniques.This is a study of a single centre of reference, comprising a retrospective and two prospective non-randomized studies.The study involved 1093 recurring patients, of whom 498 were male (45.6%) and 595 were female (54.4%) and had all been subject to ERCP and endoscopic sphincterotomy (ES) from 2002 until March 2013 in the University Gastrointestinal Clinic, in the University General Hospital of Larissa. In order for statistical significance to be achieved when comparing ERCP with LEVR, 57 additional patients were included after the end of the statistical analysis, who participated in a laparoendoscopic rendezvous from March 2013 to November 2015. The data collection for the retrospective part of the study was based on the patients’ medical records, which are found in the hospital’s archive, as well as on the clinic’s information system. The prospective part of the study is based on the analytical record of the data, which is found in the information system of the clinic.In total, 1093 patients underwent ECRP and endoscopic sphincterotomy for the first time. Complications appeared in 88 patients (8.1%). One of the most frequent complications was post-ECRP pancreatitis (3.5%), whose severity was either mild or moderate (81.5%) and there was also one death. During sphincterotomy, heamorrhage appeared in 23 patients (2.3%), while 3 patients (0.3%) suffered from late heamorrhage, which required a new endoscopy. Another complication was cholangitis in 16 patients (1.5%), which was treated with antibiotics. Moreover, there were 4 perforations (0.4%) and 2 cardiorespiratory complications (0.2%).Some high risk factors for the emergence of post-ERCP pancreatitis involve the female sex, the younger age, the flat Vater tubercle, the inconvenient intubation and the failure in intubating the gall-bladder. Pre-sphincterotomy does not increase the risk for post-ECRP pancreatitis. The training doctor’s participation does not influence neither the success rate nor the post-ECRP complications, but relates to the increased percentage of destructive complications.A precautionary dose of a 50mg diclofenac suppository 30 minutes before the examination reduces the extent and the severity of the post-ERCP pancreatitis, as opposed to the virtual medication. The difference was statistically significant regarding mainly the extent and not so much the severity of the post-ECRP pancreatitis.Cholangitis appeared in 12 patients (1.5%) with a benign disorder, of whom 4 (0.006%) had their bile duct totally cleaned, while in 8 of them (5.1%) there was an unsuccessful total cleaning. Bile duct full cleaning is the most significant factor towards the prevention of post-ERCP cholangitis. There was no difference in the percentage of post-ERCP cholangitis between the patients with a benign disorder and those with a malignant one.A pancreatic endoprosthesis was preventively used in 22 patients that had a difficult intubation. There were 3 instances of pancreatitis of mild severity (13.6%). The percentage of post-ERCP pancreatitis occurrence was similar in the cases of difficult intubation (12%), but was lower in the cases in which either a wire guide had been inserted into the pancreatic duct more than twice (23.1%) or there had been a shading of the pancreatic duct (40.4%), in patients that had received a preventive dose of diclofenac suppository. It is worth noting that there was no incidence of post-ERCP pancreatitis in the cases of pancreatic duct shading.The best way towards the prevention of complications is the patients’ right choice, so that the unintentional imaging and invasive examinations can be avoided, thus reducing both the cost and the complications and achieving a quick and effective treatment. In cases of a potential cholelithiasis, the combination of a clinical ascending cholangitis with a bile duct distension less than 10 mm from the US warns for the occurrence of cholethiasis at 85%. In the opposite case, MRCP may constitute the next diagnostic step before ERCP conduct is decided.
