Εισαγωγή: Η συσκευή ουδού εμπέδησης έχει σχεδιαστεί προκειμένου να βελτιώσει την φλεβική επιστροφή αίματος στην καρδιά, μέσω αύξησης της αρνητικής ενδοθωρακικής πίεσης κατά τη φάση της αποσυμπίεσης του θωρακικού τοιχώματος. Σκοπός: Να εκτιμήσουμε εάν η χρήση της συσκευής ουδού εμπέδησης θα βελτιώσει την ανάκτηση της αυτόματης κυκλοφορίας, την επιβίωση στις 48 ώρες και την νευρολογική έκβαση σε χοίρειο μοντέλο ανακοπής από ασφυξία. Μέθοδος: Πρόκειται για μια προοπτική, τυχαιοποιημένη, διπλή τυφλή μελέτη στην οποία μελετήθηκαν συνολικά 30 χοίροι μέσου βάρους 19±2 kg και ηλικίας 10-15 εβδομάδων. Επτά λεπτά μετά τον αποκλεισμό του ενδοτραχειακού σωλήνα, τα ζώα εμφάνισαν κοιλιακή μαρμαρυγή η οποία παρέμεινε αθεράπευτη για 8 λεπτά. Πριν από την έναρξη της καρδιοαναπνευστικής αναζωογόνησης τα ζώα τυχαιοποιήθηκαν σε δυο ομάδες. Απινιδισμός γινόταν κάθε 2 λεπτά, μέχρι την ανάκτηση αυτόματης κυκλοφορίας ή την εμφάνιση ασυστολίας. Αποτελέσματα: Ανάκτηση αυτόματης κυκλοφορίας παρατηρήθηκε σε 6 (40%) χοίρους της ομάδας ελέγχου και 14 (93,3%) της ομάδας παρέμβασης (p=0.005). Μετά από 48ωρη παρακολούθηση, 3 ζώα στην ομάδα ελέγχου επιβίωσαν σε σύγκριση με 11 στην ομάδα παρέμβασης (p<0,01). Επί πλέον, 4 ώρες μετά από την επιτυχή αναζωογόνηση παρατηρήθηκε στατιστικά σημαντική αύξηση στις τιμές των πρωτεϊνών NSE και S-100 στα ζώα της ομάδας ελέγχου σε σχέση με τα ζώα της ομάδας παρέμβασης (p<0,01). Τα ζώα της ομάδας παρέμβασης είχαν καλύτερη νευρολογική έκβαση σε σχέση με τα ζώα της ομάδας ελέγχου, 48 ώρες μετά από την ανάκτηση της αυτόματης κυκλοφορίας (79.1±18.7 έναντι 50±10, p<0.05). Συμπεράσματα: Η παρούσα μελέτη έδειξε ότι η συσκευή ουδού εμπέδησης βελτίωσε το ποσοστό ανάκτησης αυτόματης κυκλοφορίας, την 48ωρη επιβίωση, καθώς και τη νευρολογική έκβαση σε χοίρειο μοντέλο κοιλιακής μαρμαρυγής από ασφυξία.

At 48 hrs, neurologic alertness score was significantly better in animals treated with the active valve (79.1 ± 18.7 vs. 50 ± 10, p < .05) and was strongly negatively correlated with 1- and 4-hr postresuscitation neuron-specific enolase (r = -.86, p < .001 and r = -.87, p < .001, respectively) and S-100 (r = -.77, p < .001 and r = -0.8, p = .001) values.Conclusions: In this model of asphyxial cardiac arrest, intermittent airway occlusion with the impedance threshold device during the decompression phase of active compression-decompression cardiopulmonary resuscitation significantly improved hemodynamic parameters, 24- and 48-hr survival, and neurologic outcome evaluated both with clinical and biochemical parameters.
