Introduction: Due to the geography of the country and the distribution of perinatal care centers in Greece, neonatal transport (NT) becomes a necessity.Purpose: To record the main adverse events during urgent NTs as those were reported on admission to Neonatal Intensive Care Units in tertiary hospitals of Athens, the outcome of transported neonates and their correlation with different factors.Materials and methods: During the period of 1/1/2009-31/12/2009 a total of 677 urgent neonatal transports were performed. Neonates were transported from regional hospitals by ambulance or air transport, from hospitals with or without NICU in Athens and were admitted to Neonatal Intensive Care Units (NICU) in tertiary hospitals in Athens. The required data was collected with retrospective search of medical records. For statistical analysis chi-square test, McNemar test and multiple logistic regression from the SPSS-20 statistics package were used. Results: 22,2% of neonates presented with at least ...
main adverse events during urgent NTs as those were reported on admission to Neonatal Intensive Care Units in tertiary hospitals of Athens, the outcome of transported neonates and their correlation with different factors.Materials and methods: During the period of 1/1/2009-31/12/2009 a total of 677 urgent neonatal transports were performed. Results: 22,2% of neonates presented with at least one complication; hypoglycemia (glucose≤50mg/dl) 5,9%, hypothermia (temperature<36°C) 5,9% and urgent intra-venous line placement (5,2%) were most commonly observed. During transportation complications were deteriorated (27,8%) especially thermoregulation and glucose administration. Statistically important correlation with the type of transportation used, was seen only in hyperthermia’s deterioration (p=0,033), when neonates were transported from regional hospitals with an ambulance and especially if they had to travel >150km. Concerning neonatal outcome, during their hospitalization 29% had respiratory distress syndrome, 8,7% intraventricular hemorrhage and mortality was 3,4% (N=24 neonates).Regardless the type of transportation used, the highest percentage in complications and correlation with outcome, was seen in neonates with birth weight≤1500 gr, gestational age ≤32 weeks and those transported from hospitals with NICU in Attika, as those represent the majority of them. Statistically important correlation with neonatal outcome was reported with hypoglycemia (p<0.001)Conclusions: Results of this study show that neonates with birth weight≤1500 gr, gestational age ≤32 weeks, complications during pregnancy and those transported in the first day of life have greater probability to present with complications and worse outcome. Hypoglycemia and hypothermia are the most commonly observed. The recognition of these vulnerable groups and their prober stabilization with emphasis on thermoregulation and intravenous fluid administration would reduce the percentage of these complications.>
