ΕΙΣΑΓΩΓΗ: Το τραύμα είναι μια παραμελημένη , όπως χαρακτηριστικά ονομάζεται, σύγχρονη νόσος του καθημερινού άγνωστου πολέμου που λέγεται ατύχημα. Σε ελάχιστες χώρες ανεξαρτήτως από το επίπεδο ανάπτυξης τους, δεν εμφανίζονται οι τραυματισμοί μεταξύ των πέντε κύριων αιτιών θανάτου. Οι διάφορες πηγές καταγραφής των ατυχημάτων περιορίζονται σε επιδημιολογικά και συγκοινωνιολογικά στοιχεία χωρίς να επεκτείνονται σε στοιχεία που συνιστούν την ιατρική άποψη, όπως είναι το είδος και η βαρύτητα των κακώσεων, η ανατομική τους κατανομή, η έκβαση του τραυματία κλπ. ΣΚΟΠΟΣ: της παρούσας μελέτης είναι η καταγραφή και η έρευνα του τραύματος στην περιοχή μας, με αφορμή την έλλειψη αυτού του είδους στοιχείων και την επιθυμία μας να εκτιμήσουμε την υπάρχουσα κατάσταση και να διαπιστώσουμε το μέγεθος του προβλήματος και τις τυχών ιδιαιτερότητες του τραύματος στο χώρο μας, δίνοντας έμφαση στο πρόβλημα των πολυτραυματιών και των ατυχηματικών θανάτων. Σε ελάχιστες χώρες ανεξαρτήτως από το επίπεδο ανάπτυξης τους, δεν εμφανίζονται οι τραυματισμοί μεταξύ των πέντε κύριων αιτιών θανάτου. Οι διάφορες πηγές καταγραφής των ατυχημάτων περιορίζονται σε επιδημιολογικά και συγκοινωνιολογικά στοιχεία χωρίς να επεκτείνονται σε στοιχεία που συνιστούν την ιατρική άποψη, όπως είναι το είδος και η βαρύτητα των κακώσεων, η ανατομική τους κατανομή, η έκβαση του τραυματία κλπ. ΣΚΟΠΟΣ: της παρούσας μελέτης είναι η καταγραφή και η έρευνα του τραύματος στην περιοχή μας, με αφορμή την έλλειψη αυτού του είδους στοιχείων και την επιθυμία μας να εκτιμήσουμε την υπάρχουσα κατάσταση και να διαπιστώσουμε το μέγεθος του προβλήματος και τις τυχών ιδιαιτερότητες του τραύματος στο χώρο μας, δίνοντας έμφαση στο πρόβλημα των πολυτραυματιών και των ατυχηματικών θανάτων. ΥΛΙΚΟ-ΜΕΘΟΔΟΣ: Υλικό αποτελούν οι τραυματίες που διακομίσθηκαν στο ΤΕΠΑ του Γενικού Νοσοκομείου Λεμεσού το έτος 2005 και εισήχθησαν στη Χειρουργική Κλινική. Στη διάρκεια του έτους καταγράφηκαν και μελετήθηκαν στο σύνολο των τραυματιών που αντιμετωπίσαμε, βάση πρωτοκόλλου στοιχεία που προέρχονται από τις διάφορες φάσεις περίθαλψης, από τη στιγμή του τραυματισμού, τις ενέργειες στον τόπο του ατυχήματος και κατά την διακομιδή στο Νοσοκομείο, μέχρι την είσοδο στο Τμήμα Επειγόντων Περιστατικών και Ατυχημάτων, την εισαγωγή στο θάλαμο παρακολούθησης και την πλήρη αποκατάσταση του τραυματία. Οι εισαχθέντες τραυματίες για την καλύτερη μελέτη και εξαγωγή συμπερασμάτων διαχωρίσθηκαν σε 4 ομάδες αναλόγως της βαρύτητας των κακώσεων τους σύμφωνα με τον δείκτη Injury Severity Score (ISS) ως εξής: ISS 1-9, 10-15, 16-24, >24. ΑΠΟΤΕΛΕΣΜΑΤΑ: Σύμφωνα με τα υπάρχοντα στατιστικά στοιχεία στην ευρύτερη υγειονομική περιφέρεια που καλύπτει το Γενικό Νοσοκομείο Λεμεσού οι τραυματίες από τροχαία, εργατικά και άλλα ατυχήματα αντιπροσωπεύουν το 11,2% του συνολικού αριθμού ασθενών που προσήλθαν για εξέταση και το ποσοστό εισαγωγών από τροχαία στο επίπεδο της κλινικής μας αυξάνεται στο 17,9% του συνόλου των εισαγωγών. Από το σύνολο των καταγεγραμμένων τραυματιών (Ν=1070) επικράτησαν οι άνδρες νεαρής ηλικίας (25-34 ετών) που έφεραν κλειστές κακώσεις κυρίως κεφαλής, οι οποίες οφείλονταν σε τροχαία ατυχήματα. Η μεγαλύτερη προσέλευση παρατηρήθηκε τους θερινούς μήνες, αλλά βαρύτερες κακώσεις σημειώθηκαν τον χειμώνα. Για τους τραυματίες που φθάνουν την ίδια μέρα που συμβαίνει το ατύχημα στο Νοσοκομείο ο μέσος όρος χρόνου διακομιδής είναι 1 ώρα και 19 λεπτά. Ο χρόνος νοσηλείας των τραυματιών με ISS10-24 ήταν σχεδόν διπλάσιας από τον αναμενόμενο σε σχέση με αυτόν που θα αναμενόταν ανάλογα με την βαρύτητα των κακώσεων τους και σημαντική επίδραση στον παρατεταμένο χρόνο νοσηλείας φαίνεται να παίζει η ανάπτυξη επιπλοκών κυρίως εκ του αναπνευστικού. Ο προορισμός των διακομισθέντων ασθενών φανερώνει ότι οι πιο βαριά τραυματίες (ISS 16-24, >24) διακομίσθηκαν σε άλλο Νοσοκομείο λόγω έλλειψης Νευροχειρουργικής Κλινικής, στους οποίους η επιβίωση ήταν αισθητά μικρότερη από την αναμενόμενη. Ενδιαφέρον παρουσιάζει η επιβίωση των τραυματιών σε σχέση με την αναμενόμενη και συγκεκριμένα στις κατηγορίες των ασθενών με ISS10-24 (Ν=11) η θνητότητα είναι σημαντικά μεγαλύτερη από την αναμενόμενη. Περαιτέρω ανάλυση αυτών των ασθενών έδειξε ότι οι περισσότεροι από αυτούς (82%, 9/11) είχαν υποστεί ΚΕΚ παραμένοντας στο ΤΕΠ 212,5 λεπτά κατά μέσο όρο. Οι υπόλοιποι ασθενείς (18%, 2/11) κατέληξαν εξαιτίας επιπλοκών κοιλιακού και θωρακικού τραύματος. ΣΥΜΠΕΡΑΣΜΑΤΑ: Στην αντιμετώπιση του τραυματία και κυρίως του πολυτραυματία θεμελιώδους σημασίας είναι η δημιουργία στη χώρα μας ενός οργανωμένου συστήματος περίθαλψης. Ο αριθμός των τραυματιών και πολυτραυματιών σε συνδυασμό με τα πληθυσμιακά και γεωγραφικά χαρακτηριστικά της περιοχής μας συνηγορούν στην δημιουργία ενός Κέντρου Τραύματος. Με τον τρόπο αυτό θα καταστεί δυνατή η βελτίωση του συστήματος διακομιδών και της εργονομικής χωροταξίας του ΤΕΠ, η δημιουργία Ομάδας Τραύματος καθώς και χειρουργικής ΜΕΘ εξειδικευμένης σε τραύμα. Απαραίτητη προϋπόθεση είναι η ίδρυση Νευροχειρουργικού Τμήματος καθότι στη μελέτη μας σημαντικότατο πρόβλημα αποτελούν οι κρανιοεγκεφαλικές κακώσεις. Τα συμπεράσματα αυτής της μελέτης δίνουν τη δυνατότητα να γίνουν συγκρίσεις όχι μόνο με άλλες περιοχές της χώρας μας ή του εξωτερικού αλλά και με την ίδια περιοχή στο μέλλον. Αυτή η σύγκριση θα μας επιτρέψει να διαπιστώσουμε στο μέλλον αν υπήρξε κάποια βελτίωση στο υπάρχον σύστημα περίθαλψης τραυματιών ή όχι.

AIM: of the current study is the recording and investigation of trauma in our region, due to the lack of such data and our desire to evaluate the present situation and to determine the extend of the problem and the possible particularities of trauma in our region, focusing on the problem of the severely injured patients and the accidental deaths. MATERIAL-METHODS: The data of the research are based on the injured people that were presented at the Accident and Emergency Department (A&E) of the Limassol General Hospital in 2005 and were admitted to the General Surgery Department. During the year several different data were recorded and studied out of the total number of injured patients that were treated, according to a protocol, each from a different stage of treatment, from the time the accident happened, the acts at the accident’s scene and during the transport to the hospital, until the arrival at the A&E Department, the hospitalization at the Ward and the complete rehabilitation of the patient. In order to carry out a better study, the injured people were separated in 4 groups according to the severity of the injuries based on the Injury Severity Score (ISS) e.g.: ISS 1-9, 10-15, 16-24, >24. RESULTS: According to the existing statistical data, the injured patients from road traffic accidents, labour accidents and other types represent 11.2% of the total number of patients that were examined and the percentage of the injured people involved in road traffic accidents that were hospitalized at our Department, is increased to 17.9%of the total number of admissions. Out of the total number of the recorded injured patients (N=1070), the majority were males in the age 25-34 years, that had closed injuries, mainly on the head due to road traffic accident. Most of the accidents happened during the summer but the most severe ones were recorded during the winter. The average time for the transport of the injured people to the A&E Department is 1 hour and 19 minutes. The duration of hospitalization of the patients with ISS10-24 was twice the expected based on the severity of the injuries, mainly because of respiratory complications. The destination of the patients transferred to another hospital reveals that the more severely injured patients (ISS 16-24, >24) were transported to another hospital due to the lack of Neurosurgical Department, in whom the survival was significantly lower compared to the expected. The survival of the injured patients in relation with the expected, shows great interest and specifically in the category of the patients with ISS 10-24 (N=11), the mortality is significantly higher compared to the expected. Further analysis of the deaths in these two groups revealed that most of the patients (9/11), had suffered head injuries and spent a mean time of 212,5 minutes (120-425) in the Emergency Room. This could be explained by the lack of neurosurgery service in Limassol General Hospital and the need to transfer these patients to another facility. The rest of the patients (2/11) expired due to complications of complex abdominal and thoracic trauma. CONCLUSIONS: In the management of the injured and especially the severely injured patients, it is of great importance the foundation of an organized system of treatment in our country. The number of injured and severely injured patients in correlation with the population and geographic characteristics of our region explain the need for creating a Trauma Center. In this way, the system of patient transport can be imported together with the work and space allocation at the A&E Department, the activation of the Trauma Team as well as the Surgical ICU specialized in trauma. An absolute prerequisite, is the foundation of a Neurosurgery Department since in our study, one of the greatest problem is the head injuries. The conclusions of this study, allow comparisons to be made, not only with other regions of our country or abroad, but with the same region in the future. This will enable us in the future to note whether there has been any improvement in the existing system of trauma care or not.
