Η ερυθροποιητίνη είναι μία κυτταροκίνη που παίζει σημαντικό ρόλο στην παραγωγή και απόπτωση των ερυθρών αιμοσφαιρίων. Η Epo δρα μέσω των υποδοχέων της που εκφράζονται κυρίως στα πρόδρομα κύτταρα της ερυθράς σειράς CFU-E, αλλά και σε κύτταρα του ΚΝΣ και της καρδιάς. Η δράση της Epo και του Epo-R γίνεται μέσω της οδού JAK-STAT κατά την οποία το μήνυμα φτάνει τελικά στον πυρήνα του κυττάρου. Εκεί επιδρά στο γονίδιο bcl-x, που επηρεάζει την απόπτωση των κυττάρων της ερυθράς σειράς. Σήμερα γνωρίζουμε ότι στην παθογένεια των ΜΥΣ που είναι αρνητικά στο bcr/abl, η ΑΠ, η ΙΘ και η ΙΜ, όπως και η οικογενής πολυερυθραιμία παίζει σημαντικό ρόλο η επίδραση της Epo, του Epo-R και της JAK 2 κινάσης. Η οικογενής πολυερυθραιμία αποτελεί μία μορφή πρωτοπαθούς πολυερυθραιμίας. Διαφέρει από την ΑΠ τόσο μορικά, όσο και εργαστηριακά και κλινικά. Θεωρείται ότι η παθογένειά της οφείλεται κατά μεγάλο μέρος στις μεταλλάξεις του Epo-R. Αυτές οδηγούν σε υπέρμετρη παραγωγή της ερυθράς σειράς και με την αντίστοιχη κλινική συμπτωματολογία. Μέχρι σήμερα έχουν αναφερθεί 13 μεταλλάξεις του Epo-R, από τις οποίες οι 10 σχετίζονται άμεσα με την οικογενή πολυερυθραιμία. Εμείς προσπαθήσαμε να ανιχνεύσουμε μεταλλάξεις του EpoR σε μέλη και συγγενείς οικογενειών με διαγνωστικά κριτήρια οικογενούς πολυερυθραιμίας, χωρίς όμως να έχουμε κάποιο θετικό αποτέλεσμα. Η μοριακή δράση της Epo γίνεται μέσω του Epo-R και της JAK 2 κινάσης και προς την κατεύθυνση αυτή, και με βάση τη διεθνή βιβλιογραφία, ερευνήσαμε τους ασθενειών μας που έπασχαν από οικογενή πολυερυθραιμία και ΜΥΣ ως προς την μετάλλαξη JAK 2V617F. Η μετάλλαξη αυτή είναι προϊόν έρευνας των τριών τελευταίων ετών και φαίνεται ότι ρίχνει αρκετό φως στην παθογένεια των bcr/abl αρνητικών ΜΥΣ. Η V617F συμβαίνει στην περιοχή του cDNA που κωδικοποιεί την ψευδοκινάση (JH2 περιοχή) και συγκεκριμένα στη θέση 1849 έχουμε αντικατάσταση του νουκλεοτιδίου της γουανίνης (G) από τη θυμίνη (T). Αυτή η αλλαγή οδηγεί σε αντικατάσταση του αμινοξέος βαλίνη από τη φαινυλαλανίνη (Val617Phe,V617F), στο JH2 τμήμα του μορίου της JAK 2. Αυτό έχει σαν αποτέλεσμα την αναστολή της δράσης της JAK 2 από το JH2 τμήμα, που οδηγεί σε αυξημένη δραστηριότητα της κινάσης, ακόμα και σε απουσία της Εpο, με τελικό αποτέλεσμα την αναστολή της απόπτωσης και τον αυξημένο πολλαπλασιασμό των πρόδρομων κυττάρων του αίματος. Κάτι αντίστοιχο, δηλαδή με τη βλάβη στη ΧΜΛ, μέσω του γονιδίου bcr/abl. Σήμερα η μετάλλαξη V617F ανευρίσκεται σε ποσοστό μεγαλύτερο του 95% στην ΑΠ, και σε ποσοστό περίπου 50% στην ΙΘ και την ΙΜ. Πιθανολογείται ότι η παρουσία της μετάλλαξης στα ΜΥΣ επηρεάζει την κλινική εικόνα της ΑΠ και της ΙΘ, κυρίως όσον αφορά στα επεισόδια θρομβώσεων και αιμορραγιών, όπως και την μετάπτωση σε ΙΜ και οξεία Λευχαιμία. Στην παρούσα εργασία δεν ανιχνεύθηκε η JAK 2 V617F στην οικογενή πολυερυθραιμία. Στα ΜΥΣ βρέθηκε μετάλλαξη σε ποσοστά 62% για την ΑΠ, 45% για την ΙΘ και 60% για την ΙΜ. Επιπλέον, βρέθηκε στατιστικά σημαντική θετική συσχέτιση του αριθμού των αιμοπεταλίων και της παρουσίας της V617F στους ασθενείς με ΑΠ. Στους ίδιους ασθενείς με την εμφάνιση θρομβωτικών και αιμορραγικών επεισοδίων βρέθηκε θετική συσχέτιση με την μετάλλαξη. Η διαπίστωση ότι οι ασθενείς με ΑΠ βρέθηκε να είναι ομοζύγοι ως προς την V617F ποσοστό 22% για τη μετάλλαξη, στοιχείο που επιβεβαιώνεται από τις τρέχουσες δημοσιεύσεις, δείχνει τη δράση του μιτωτικού ανασυνδυασμού του DNA και την ύπαρξη υποκλώνων μεταλλαγμένων κυττάρων. Αντίθετα, σε ασθενείς με ΙΘ και ΙΜ δεν ανιχνεύθηκε ομοζυγωτία για τη μετάλλαξη. Η μετατροπή της ομόζυγης μορφής Y617F μετάλλαξης σε ετερόζυγη, σε ασθενή με ΑΠ, μετά από χορήγηση ιντερφερόνης, δείχνει τη σημαντική δράση του φαρμάκου στα ΜΥΣ, αλλά και τη δυνατότητα που υπάρχει για θεραπευτική προσέγγιση της νόσου. Σήμερα γίνεται προσπάθεια θεραπείας των ΜΥΣ μέσω αναστολής της δράσης τηςJAK 2 κινάσης αλλά ακόμα η έρευνα βρίσκεται σε αρχικά στάδια. Τελικά φαίνεται ότι η JAK 2 κινάση ανοίγει νέους δρόμους όσον αφορά στην κατανόηση της παθογένειας των ΜΥΣ, την διάγνωση, την ταξινόμηση, την πρόγνωση και την ανταπόκριση στη θεραπεία με τα υπάρχοντα φάρμακα αλλά και τη δημιουργία νέων σκευασμάτων, που στόχο θα έχουν την αναστολή της δράσης της JAK 2 κινάσης, την καλύτερη πρόγνωση και φυσική πορεία των νοσημάτων αυτών και, ίσως, μελλοντικά την πλήρη ίασή τους.

It is though that these mutations lead to elevated proliferation of the red cell progenitors and to the characteristic clinical presentation. Until today, 13 mutations of the Epo-R have been described, from which, 10 are directly connected with the pathogenesis of Familial Polycythemia. In this study, we tried to trase any of Epo-R mutations in Greek members with diagnostic criteria for the disease and their first degree relatives. Unfortunately we did not succeed in discovering any of those mutations. As the Epo’s molecular activity is performed through the Epo-R and the JAK 2 kinase, and guided by the international literature, we investigated our patients suffering from both MDS and Familial Polycythemia, in terms of the presentation of the JAK 2 V617F mutation. In 2005 this mutation was detected in the majority of patients with PV and in half patients with IT and IM and seems to be the key for the pathogenesis ofthe bcr/abl negative MPDs. The V617F mutation takes place in the in the cDNA area that codes the pseudokinase (JH2 homologue domain) of the JAK 2 molecule. In the 1849 position there is a substitution of the nucleoside guanine (G) to thymine (I). This change leads to the substitution of the aminoacid valine to phenylalanine in the position 617 in JFI2 domain of JAK 2 molecule (Val617Phe, V617F). This mutation leads to downregulatoin of action of JH2 domain, which normally inhibits the JAK 2 kinase activity. In such a case, increased kinase activity is observed even in the absence of Epo. The final result is the inhibition of apoptosis and the proliferation of the blood cell precursors, more of less the same disorder that happens in CML, by the action of bcr/abl. Today, the JAK 2 V617F mutation is detected in more than 95% of the PV patients and in almost half of the IT and IM patients. It is widely suspected that the presense of the mutation affects the clinical manifestations of the PV and IT, mainly expressed by the high incidence of throbotic and hemorrhagic episodes, as well as the transition to IM and acute Leukemia. In this study we did not detect any mutation of JAK 2 V617F in the familial polycythemia patients. Besides, the mutation was found in 62% of PV patients, in 45% of IT patients and in 60% for IM patients. Furthermore, we noticed a significant positive statistical connection between the number of platelets and the presence of V617F in PV patients. The same patients expressed positive correlation in the coincidence of thrombotic and hemorrhagic episodes and the presence of V617F mutation. The finding of homozygosity of JAK 2 V617F in 22% of PV patients, which is conformed by multiple references, reveals the activity of the mitotic recombination of DNA and the presence of mutant subclones. On the other hand, there were no homozygous IT and IM patients. The transformation of homozygous form of V617F into eterozygous, in a single PV patient, after the use of interferon, reveals the theurapetic action of this molecule in the MDS and the possibility of high molecular and clinical response after treatment. There are some attempts of management of MPDs, through the inhibition of JAK 2 kinase activity, but we are still at beginning of our efforts. Finally, it is sure that JAK 2 kinase is probably the key molecule in discovering and understanding the molecular pathogenesis of MPDs. It is more than helpful in achieving diagnosis, prognosis and theurapetic response of MPDs. Now we have the opportunity to reach a theurapeutic management of the diseases using the current therapies, or discovering new regimens that change the natural history and prognosis of MPDs.
