The role of operative laparoscopy in the management of infertility due to endometriosis

Abstract

Introduction: Advancement in techniques and instrumentation for operative laparoscopy has allowed the performance of many gynecologic procedures that they have previously required laparotomy. This study compared the surgical management and pregnancy rates in women with advanced endometriosis in endometriomas management surgery versus laparotomy. Methods: The patients polulation consisted of 268 infertile women suffering from extensive endometriosis with endometriomas. The endometriotic lessions were staged in both groups (laparotomy, laparoscopy) according to the rAFS scoring system in 1985. From the included patients 140 underwent microsurgery laparotomy while the remaining 128 underwent advanced operative laparoscopy. All women were followed up for the time of operation until they become pregnant, or at least 24 months if they did not.
