Το Clostridioides difficile (C. difficile) είναι ένα από τα σημαντικότερα νοσοκομειακά παθογόνα, και η λοίμωξη που προκαλεί συνδέεται με διαταραχές στο εντερικό μικροβίωμα. Η μετάδοση του C. difficile πραγματοποιείται μέσω της κοπρανο-στοματικής οδού. Η έκθεση σε αντιβιοτικά είναι ο κύριος παράγοντας κινδύνου για την ανάπτυξη λοίμωξης από C. difficile - CDI (Clostridioides difficile infection), καθώς διαταράσσει τη φυσιολογική σύνθεση και λειτουργία του εντερικού μικροβιώματος. Η ανάπτυξη της CDI ευνοείται από τις διαταραχές στο μικροβίωμα του εντέρου, ενώ η αποκατάσταση της ποικιλομορφίας και της αφθονίας των ωφέλιμων βακτηρίων του εντερικού μικροβιώματος αποτρέπει την ανάπτυξή της. Η παρούσα μελέτη στοχεύει στη χαρτογράφηση του μικροβιώματος του εντέρου τόσο σε υγιή άτομα όσο και σε ασθενείς με CDI. Τα αποτελέσματά της θα αναλυθούν και θα συγκριθούν σε σχέση με το φύλο. Επιπλέον ο πυρήνας του εντερικού μικροβιώματος των δύο ομάδων της μελέτης θα συγκριθεί για να προσδιοριστούν τυχόν αλλαγές στο εντερικό μικροβίωμα σε άτομα με επιβεβαιωμένη CDI.Στη μελέτη συμπεριλήφθηκαν 48 άτομα, κάτοικοι της Βόρειας Ελλάδας και ηλικίας άνω των 18 ετών. Οι συμμετέχοντες ταξινομήθηκαν σε δύο ομάδες: 16 υγιή άτομα και 32 ασθενείς με CDI, δηλαδή νοσηλευόμενοι ασθενείς με διάρροια που βρέθηκαν θετικοί σε τοξινογόνο C. difficile. Τα δείγματα κοπράνων των ασθενών συλλέχθηκαν μετά την εμφάνιση της διάρροιας. Η διάγνωση της CDI έγινε με ταχεία ανίχνευση του τοξινογόνου C. difficile και επιβεβαιώθηκε με συνδυασμό ανοσοενζυμικών, καλλιεργητικών και μοριακών μεθόδων. Το μικροβίωμα αναλύθηκε με αλληλούχιση των υπερμεταβλητών περιοχών του γονιδίου 16S rRNA (16S ribosomal RNA) σε σύστημα Ion GeneStudio™ S5. Για κάθε δείγμα, χρησιμοποιήθηκαν δύο σετ εκκινητών, V2-4-8 και V3-6, 7-9, τα οποία επιτρέπουν την ακριβή αναγνώριση και ταυτοποίηση ενός ευρέος φάσματος βακτηρίων σε επίπεδο γένους και είδους. Περαιτέρω στατιστική ανάλυση των δεδομένων του μικροβιώματος πραγματοποιήθηκε με τη γλώσσα προγραμματισμού ανοιχτού κώδικα R, έκδοση 3.5.2. Μεταξύ των ασθενών με CDI, τα φύλα με τη μεγαλύτερη αφθονία ήταν τα Firmicutes και Proteobacteria , ενώ οι Enterobacteriaceae και Enterococcaceae ήταν οι πιο άφθονες οικογένειες. Η ανάλυση σε επίπεδο γένους έδειξε ότι το γένος Enterococcus ήταν το κυρίαρχο γένος στην ομάδα των ασθενών με CDI, σε αντίθεση με τα υγιή άτομα, στα οποία ήταν το Faecalibacterium. Περαιτέρω ανάλυση με το εργαλείο MaAsLin2 (Microbiome Multivariable Association with Linear Models) έδειξε ότι η οικογένεια Enterococcaceae και το γένος Enterococcus ήταν σχετικά πιο άφθονα σε ασθενείς με CDI σε σχέση με τα υγιή άτομα επιβεβαιώνοντας τα ευρήματα της διερεύνησης αφθονίας. Η ανάλυση της α-ποικιλομορφίας και της β-ποικιλομορφίας δεν έδειξε διαφορές μεταξύ των ασθενών με CDI και των υγιών ατόμων. Ο έλεγχος για την αφθονία και για την ποικιλομορφία ανάμεσα στα δύο γενετικά φύλα στην ομάδα των ασθενών δεν εμφάνισε διαφορές. Συμπερασματικά, η χαρτογράφηση του μικροβιώματος στις δύο ομάδες της μελέτης έδειξε ότι υπάρχουν διαφορές του εντερικού μικροβιώματος μεταξύ υγιών ατόμων και ασθενών με CDI. Ωστόσο, απαιτούνται περαιτέρω μελέτες με τη συμμετοχή περισσότερων ατόμων, προκειμένου να διερευνηθεί ο ενδεχόμενος ρόλος του εντερικού μικροβιώματος ως προγνωστικός βιοδείκτης για την εκδήλωση της CDI.

C. difficile is transmitted via the fecal-oral route. Exposure to antibiotics is the primary risk factor for the development of Clostridioides difficile infection (CDI), as it disrupts the normal composition and function of the gut microbiome. The participants were divided into two groups: 16 healthy individuals and 32 patients with CDI, that is, hospitalized patients with diarrhea who tested positive for C. difficile toxin. Fecal samples from the patients were collected after the onset of diarrhea. CDI was diagnosed using rapid detection of toxin-producing C. difficile and confirmed using a combination of immunoenzymatic, culture, and molecular methods. The microbiome was analyzed by sequencing the hypervariable regions of the 16S rRNA (16S ribosomal RNA) gene on an Ion GeneStudio™ S5 system. For each sample, two sets of primers were used—V2-4-8 and V3-6, 7-9—which allow for the precise identification and characterization of a broad spectrum of bacteria at the genus and species levels. Further statistical analysis of the microbiome data was performed using the open-source programming language R, version 3.5.2. Among patients with CDI, the most abundant phyla were Firmicutes and Proteobacteria, while Enterobacteriaceae and Enterococcaceae were the most abundant families. Analysis at the genus level showed that the genus Enterococcus was the dominant genus in the group of patients with CDI, in contrast to healthy individuals, in whom Faecalibacterium was the dominant genus. Further analysis using the MaAsLin2 (Microbiome Multivariable Association with Linear Models) tool showed that the Enterococcaceae family and the Enterococcus genus were relatively more abundant in patients with CDI compared to healthy individuals, confirming the findings of the abundance analysis. Analysis of α-diversity and β-diversity revealed no differences between patients with CDI and healthy individuals. An assessment of abundance and diversity between the two biological sexes within the patient group showed no differences.In conclusion, the microbiome analysis of the two study groups showed differences in the gut microbiome between healthy individuals and patients with CDI. However, further studies involving a larger number of participants are needed to investigate the potential role of the gut microbiome as a prognostic biomarker for the onset of CDI.
