Ο όρος σύνδρομο θωρακικής εξόδου (Σ.Θ.Ε) χρησιμοποιείται για να περιγράψει τη συμπτωματολογία που οφείλεται στη συμπίεση του βραχιονίου πλέγματος και των υποκλείδιων αγγείων στην περιοχή της θωρακικής εξόδου. Η πολυπλοκότητα της παθοφυσιολογίας του συνδρόμου, η απουσία αξιόπιστης και πλήρως τεκμηριωμένης διαγνωστικής δοκιμασίας, καθώς και ο κίνδυνος σοβαρών επιπλοκών, καθιστούσαν διαχρονικά αμφιλεγόμενη τη χειρουργική του αντιμετώπιση. Η διαμασχαλιαία προσπέλαση παρέχει τη δυνατότητα πλήρους εκτομής της πρώτης πλευράς σε συνδυασμό με σκαληνοτομή, με ικανοποιητικό αισθητικό αποτέλεσμα και χαμηλό ποσοστό σοβαρών επιπλοκών, όταν εφαρμόζεται από έμπειρη χειρουργική ομάδα. Σκοπός της παρούσας διδακτορικής διατριβής είναι η συστηματική αξιολόγηση των πρώιμων και απώτερων αποτελεσμάτων της διαμασχαλιαίας πρόσθιας σκαληνοτομής σε συνδυασμό με εκτομή της πρώτης πλευράς ως οριστικής θεραπείας του Σ.Θ.Ε , καθώς και η διεγχειρητική ηλεκτροφυσιολογική αξιολόγηση της αποσυμπίεσης του βραχιονίου πλέγματος. Η μελέτη σχεδιάστηκε με αναδρομικό και προοπτικό σκέλος, αξιοποιώντας δεδομένα ασθενών από το Γενικό Νοσοκομείο Αττικής «ΚΑΤ» από το 1999 και εξής. Οι ασθενείς υποβλήθηκαν σε κλινικό, απεικονιστικό και ηλεκτροφυσιολογικό έλεγχο σε προκαθορισμένα χρονικά διαστήματα (3, 6, 12 και 24 μήνες), ενώ πραγματοποιήθηκε και διεγχειρητική καταγραφή της αποσυμπίεσης του βραχιονίου πλέγματος. Τα αποτελέσματα της μελέτης καταδεικνύουν ότι η συγκεκριμένη χειρουργική τεχνική αποτελεί αποτελεσματική και ασφαλή θεραπευτική επιλογή. Παρατηρήθηκε σημαντική και κλινικά ουσιώδης μείωση της συμπτωματολογίας, καθώς και βελτίωση της λειτουργικότητας και της ποιότητας ζωής των ασθενών, ήδη από την πρώιμη μετεγχειρητική περίοδο, με διατήρηση των αποτελεσμάτων σε βάθος χρόνου. Η πλειονότητα των συμπτωμάτων παρουσίασε σημαντική μείωση, ενώ εξαίρεση αποτέλεσε το άλγος της άκρας χειρός, το οποίο εμφάνισε αύξηση χωρίς να αναιρεί τη συνολική θετική έκβαση. Παραισθησίες και άλγος της ωμικής ζώνης παρέμειναν ως τα συχνότερα υπολειπόμενα συμπτώματα, ιδιαίτερα σε ασθενείς με μακρά διάρκεια συμπτωματολογίας. Σημαντική βελτίωση καταγράφηκε τόσο στις κλινικές δοκιμασίες όσο και στα απεικονιστικά ευρήματα, με σχεδόν πλήρη αποκατάσταση των ευρημάτων του έγχρωμου υπερηχογράφηματος (triplex) αγγείων άνω άκρου. Επιπλέον, παρατηρήθηκε ουσιαστική βελτίωση στις κλίμακες λειτουργικότητας και ποιότητας ζωής (CBSQ και DASH), καθώς και σημαντική μείωση της έντασης του πόνου (BPI), με περαιτέρω ήπια βελτίωση κατά την παρακολούθηση. Η προεγχειρητική φυσικοθεραπεία δεν επηρέασε το τελικό αποτέλεσμα, αλλά διαφοροποίησε το χρόνο εμφάνισης της βελτίωσης. Οι ασθενείς που υποβλήθηκαν σε φυσικοθεραπεία παρουσίασαν ταχύτερη πρώιμη αποκατάσταση, ενώ όσοι δεν υποβλήθηκαν εμφάνισαν βραδύτερη αλλά προοδευτική βελτίωση. Αντίστοιχα, στα ηλεκτροφυσιολογικά δεδομένα, οι ασθενείς χωρίς φυσικοθεραπεία παρουσίασαν στατιστικώς σημαντική αύξηση των τιμών του μέσου αισθητικού και μέσου κινητικού νεύρου, υποδηλώνοντας προοδευτική νευρική αποκατάσταση, ενώ στους ασθενείς με φυσικοθεραπεία οι τιμές παρέμειναν σταθερές. Η ανάλυση των ηλεκτροφυσιολογικών ευρημάτων ανέδειξε διαφοροποίηση μεταξύ αισθητικών και κινητικών νεύρων. Το μέσο αισθητικό νεύρο παρουσίασε αρχικά βελτίωση χωρίς περαιτέρω εξέλιξη, ενώ το ωλένιο αισθητικό νεύρο δεν εμφάνισε σημαντικές μεταβολές. Αντίθετα, τα κινητικά νεύρα παρουσίασαν προοδευτική και στατιστικώς σημαντική βελτίωση, υποδηλώνοντας μεγαλύτερη ικανότητα αποκατάστασης της κινητικής λειτουργίας. Οι μετεγχειρητικές επιπλοκές ήταν περιορισμένες και αντιμετωπίσιμες, με συχνότερη τον πνευμοθώρακα, χωρίς καταγραφή σοβαρών επιπλοκών. Παράλληλα, η διεγχειρητική νευροπαρακολούθηση ανέδειξε σαφή βελτίωση της νευρομυϊκής λειτουργίας μετά την πλήρη αποσυμπίεση, με σημαντική αύξηση της μυϊκής δραστηριότητας στη τελική ουδέτερη θέση. Αντίθετα, οι καταγραφές στο στάδιο της πρόσθιας σκαληνοτομής δε συνοδεύτηκαν από αντίστοιχες μεταβολές, υπογραμμίζοντας το καθοριστικό ρόλο της εκτομής της πρώτης πλευράς. Συμπερασματικά, η διαμασχαλιαία εκτομή της πρώτης πλευράς με πρόσθια σκαληνοτομή αποτελεί μια τεκμηριωμένη, ασφαλή και αποτελεσματική μέθοδο για την αντιμετώπιση του συνδρόμου θωρακικής εξόδου. Η μέθοδος οδηγεί σε σημαντική και διατηρούμενη βελτίωση της κλινικής εικόνας, της λειτουργικότητας και της ποιότητας ζωής των ασθενών. Η προεγχειρητική φυσικοθεραπεία επιταχύνει την πρώιμη αποκατάσταση, ενώ η νευροφυσιολογική ανάκαμψη εξελίσσεται σταδιακά και διαφοροποιείται μεταξύ αισθητικών και κινητικών νεύρων. Η διεγχειρητική νευροπαρακολούθηση αποτελεί πολύτιμο εργαλείο για την αντικειμενική αξιολόγηση της αποσυμπίεσης. Τα ευρήματα αυτά αναδεικνύουν τη σημασία της συνδυασμένης κλινικής και ηλεκτροφυσιολογικής αξιολόγησης και υπογραμμίζουν την ανάγκη για περαιτέρω μελέτες με μεγαλύτερα δείγματα και μακροχρόνια παρακολούθηση.

The surgical methods aim at decompression and functional restora-tion of the mentioned structures and include the scalenotomy and /or partial or to-tal removal of the first rib and the removal of co- existing cervical rib. The surgical methods mentioned above consist of the transaxillary , supraclavicular and subcla-vicular approach, posterior thoracoplasty and the dual approach ,which combines the transaxillary with a posterior incision and the thoracoscopic approach. The transaxillary approach provides better results, offering the possibility of scalenotomy and complete removal of the first rib, avoiding the insertion to the pleural cavity, en-suring satisfactory cosmetic result and avoiding serious complications due to injury of the subclavian vessels and the brachial plexus, provided that is performed by an experienced surgical team. The purpose of the elaborated Doctoral Thesis is the study of the early and long – term results of the transaxillary scalenotomy and removal of the first rib, as a defini-tive treatment of the Thoracic Outlet Syndrome and the intraoperative electrophysi-ological evaluation of the decompression of the brachial plexus. The need of study-ing and recording the early and especially the long –term results arises from the lack of international correlation of the respective clinical and laboratory records regard-ing the postoperative condition of the patients. The study will lead to the evaluation of this surgical method and the investigation of the appropriate time of surgery, by comparing the results of patients who underwent surgical treatment at different times after the diagnosis of the T.O.S. In addition, postoperative clinical presentation of patients who underwent surgery after completing physiotherapy sessions will be compared with patients’ presentation who underwent surgery without undergoing these sessions preoperatively. To conduct the research study, the medical records of the patients of General Hospi-tal of Attica «KAT» and specifically of General Thoracic Surgery Department and 3rd Orthopedic Clinic of National and Kapodistrian University of Athens, from 1999 on-wards, will be used. Patients will undergo clinical maneuvers, radiological examina-tion, Triplex Sonography of the subclavian vessels and evaluation of the function of the brachial plexus, by recording the time interval after the surgery. The above ex-aminations will be performed at intervals of 3, 6, 12, 24 months, adjusted to the dates of surgery in collaboration with the Vascular Ultrasound Department of the Radiology Department and Neurophysiology Laboratory of Clinic for Physical Medi-cine and Rehabilitation of the General Hospital of Attica «KAT». In addition, in-traoperative measurement, records and evaluation of the function of the brachial plexus will be performed before, during and after its surgical decompression. The epidemiological correlation of patient characteristics will be also included in the pro-tocol as well. This research study will contribute to the qualitative and quantitative evaluation of the described surgical method as the definitive treatment of the syn-drome, by filling gaps in the recording of early and long – term results and will exam-ine the direct intraoperative effect of this method on the decompression of the bra-chial plexus. The present research on one section will be retrospective and prospec-tive on the other one. Regarding its retrospective character, the patients’ medical records will be studied from 1999 until now. The patients will be categorized accord-ing to the type of T.O.S for which they were treated and according to the time peri-od, that has passed since their surgical treatment. The history of the patients will be taken and evaluated, with great emphasis on the duration of the symptoms before the surgery but also on the execution or not of preoperative physiotherapy ses-sions, as well as the total number of them. A key-role in the present study will play the epidemiological data of the patients and more specifically their profession, sports activities, as well as past injuries with concomitant injuries of the affected arm and the cervical spine. The preoperative examination of patients will be further studied. The study will also include the completion of questionnaires about the func-tionality of the affected arm and the intensity of pain preoperatively, as well as the results of the clinical maneuvers attempted. The intraoperative evaluation of the function of the brachial plexus during the various phases of the operation using elec-trodes for recording aesthetic and motor stimuli will be also included in the prospec-tive study above. Postoperative follow-up of patients will take place over a period of 3, 6, 12, 24 months as in the retrospective part of the study. The study will include patients with T.O.S, who were treated surgically through the transaxillary approach. The time period for completion of the Doctoral Thesis is estimated at three to six years. The present study systematically evaluated the early and long-term outcomes of transaxillary anterior scalenotomy combined with first rib resection for the treat-ment of T.O.S, demonstrating that this surgical approach constitutes an effective and safe therapeutic option. Overall, a significant and clinically meaningful reduction in symptomatology was observed, along with improvement in patients’ functional status and quality of life, evident from the early postoperative period and sustained over long-term follow-up. Specifically, a marked reduction in the frequency of most symptoms was recorded within the first postoperative months, which remained stable over time. An excep-tion was pain in the hand, which increased postoperatively but did not negate the overall favorable outcome. Paresthesia and shoulder girdle pain remained the most common residual symptoms, suggesting that, despite substantial improvement, complete symptom resolution is not achieved in all patients, particularly in those with longstanding disease. The surgical intervention was associated with significant improvement in both clini-cal provocative tests and imaging findings, with a notable reduction in positive tests and near-complete normalization of upper limb vascular duplex (triplex) findings, confirming the effectiveness of neurovascular decompression. Regarding functional outcomes and quality of life, significant improvements were observed in the CBSQ (Cervical Brachial Symptom Questionnaire) and DASH (Disabili-ties of the Arm, Shoulder and Hand) questionnaires, primarily evident in the early postoperative period and maintained throughout follow-up. Similarly, pain intensity, as assessed by the BPI (Brief Pain Inventory) scale, was significantly reduced after surgery, with further mild but steady improvement over time, particularly in pain severity. Preoperative physiotherapy emerged as an important factor influencing the tem-poral pattern of recovery rather than the final outcome. Patients who underwent physiotherapy demonstrated an early and statistically significant improvement in function during the initial postoperative period, which subsequently plateaued. In contrast, patients without preoperative physiotherapy showed a slower, progressive improvement that reached statistical significance at later time points. These findings suggest that physiotherapy accelerates early clinical recovery without altering the overall outcome. This differential effect was also reflected in neurophysiological data. Patients with-out preoperative physiotherapy exhibited a statistically significant increase in medi-an sensory and motor nerve values during follow-up, indicating progressive neural recovery following decompression. Conversely, patients who had undergone physio-therapy showed stable neurophysiological values, supporting the notion that physio-therapy primarily facilitates functional adaptation and early symptom relief, while neural recovery follows a slower, biologically determined course. Electrophysiological findings revealed a differential pattern between sensory and motor nerves. The median sensory nerve demonstrated significant postoperative improvement without further progression over time and a tendency to return to-ward baseline values at two years, whereas the ulnar sensory nerve showed no sig-nificant changes. In contrast, motor nerves exhibited a clear and statistically signifi-cant progressive improvement over time, both for the median and ulnar nerves, in-dicating substantial recovery of motor function following surgical decompression. The ulnar motor nerve at Erb’s point showed an increasing trend that did not reach statistical significance. Overall, these findings suggest that motor nerve function has a greater capacity for recovery compared to sensory function, while neural recovery remains partial and gradual.Postoperative complications were limited and manageable, with pneumothorax be-ing the most common. No major complications, such as phrenic nerve injury or inju-ry to the brachial plexus or subclavian vessels, were observed. However, the occur-rence of minor nerve injuries (e.g., musculocutaneous nerve involvement) in a small proportion of patients highlights the importance of surgical expertise and meticu-lous technique.Intraoperative neurophysiological monitoring further substantiated the effective-ness of surgical decompression. Decompression of the brachial plexus, achieved through the combination of anterior scalenotomy and first rib resection, was associ-ated with a clear and measurable improvement in neuromuscular function. This im-provement was primarily reflected by a statistically significant increase in muscle ac-tivity in the final neutral position, observed consistently across most examined mus-cles, indicating restoration of neural conduction following complete decompression. In contrast, intermediate stages of the procedure, particularly isolated anterior sca-lenotomy, were not associated with comparable changes, underscoring the critical role of first rib resection in achieving complete and effective decompression. Analysis of clinical and demographic factors demonstrated that intraoperative neu-rophysiological response was largely independent of variables such as preoperative physiotherapy, physical activity, or concomitant cervical spine pathology. Converse-ly, symptom duration appeared to selectively affect certain muscle groups without negating the overall positive impact of surgery, highlighting the importance of early intervention. Finally, the statistically significant correlations observed among different muscle groups across all measurement stages indicate a coordinated and systematic recov-ery of brachial plexus function, reflecting its integrated neurophysiological behavior.In conclusion, transaxillary first rib resection combined with anterior scalenotomy represents a safe, effective, and well-documented treatment for thoracic outlet syndrome. The procedure leads to substantial and sustained improvement in symp-toms, functional capacity, and quality of life. Preoperative physiotherapy primarily accelerates early clinical recovery, whereas neurophysiological recovery progresses gradually and differs between sensory and motor nerves. Intraoperative neurophys-iological monitoring emerges as a valuable tool for objectively assessing the effec-tiveness of decompression. These findings highlight the importance of integrating clinical and electrophysiological evaluation and underscore the need for further studies with larger cohorts and longer follow-up to better define the factors influ-encing treatment outcomes.
