Comparison of suppression and stimulation test of the renin-angiotensin-aldosterone system for the diagnosis of primary aldosteronism in patient with clinical suspicion of the disease

Abstract

Objective: Primary aldosteronism is one of the most common cases of secondary hypertension, accounting for more than 6% in hypertensive patients. Patients with this form of secondary hypertension are in high risk of cardiovascular disease, comorbidity, mortality and they feature extended target organ damage. The prompt and adequate diagnosis of primary aldosteronism can result in significant clinical benefits for the patients through the implementation of the proper treatment: medical treatment with mineralocorticoid receptor antagonists for bilateral disease or adrenalectomy of the affected adrenal gland for the unilateral forms. However, the available tests for the confirmation of primary aldosteronism include suppression tests of the renin-angiotensin-aldosterone axis, that they have significant limitation.
