THE PROPOSED MYOCARDIAL MECHANICAL EFFICIENCY INDEX (ΔΜΑ) CAN BE ESTIMATED NON INVASIVELY, UTILIZING THOSE "HEMODYNAMIC" PARAMETERS THAT AN EXERCISE TOLERANCE TEST (ETT) PROVIDES, FROM THE FOLLOWING FORMULA: ΔΜΑ = (VO2 MAX ACHIEVED)2 DIVIDED BY (MVO2 MAX ACHIEVED . VO2 MAX PREDICTED), BASED ON THE BRUCE AND KITAMURA REGRESSION EQUATIONS (FOR ESTIMATING VO2 MAX ACHIEVED-PREDICTED AND MVO2MAX ACHIEVED, RESPECTIVELY). 152 MALES AND 38 FEMALES UNDERGONE BOTH ETT AND CORONARY ARTERIOGRAPHY, FORMING 3 GROUPS OF DISEASE [ 1) "NORMALS", "Φ". 2) HYPERTENSIVES WITHOUT CORONARY ARTERY DISEASE (CAD) "Y". 3) CAD PATIENTS "Σ"] AND ΔΜΑ, ST CHANGES, ST SCORE [ST(MM)/ΔΜΑ] WERE CALCULATED. THE STATISTICAL ANALYSIS (T-TEST) PROVIDED THE FOLLOWING RESULTS: A) ΔΜΑ WAS STATISTICAL SIGNIFICANTLY REDUCED IN THE CAD GROUP COMPARING TO THE "Φ" AND THIS INFLUENCE WAS MOREPRONOUNCED AS THE ANGIOGRAPHIC EXTENT OF DISEASE INCREASED (3 VESSEL DISEASE = LOWEST VALUES). THE STATISTICAL ANALYSIS (T-TEST) PROVIDED THE FOLLOWING RESULTS: A) ΔΜΑ WAS STATISTICAL SIGNIFICANTLY REDUCED IN THE CAD GROUP COMPARING TO THE "Φ" AND THIS INFLUENCE WAS MOREPRONOUNCED AS THE ANGIOGRAPHIC EXTENT OF DISEASE INCREASED (3 VESSEL DISEASE = LOWEST VALUES). TREATMENT WITH B-ADRENERGIC BLOCKERS IMPROVED THE ΔΜΑ VALUESTOWARDS THE "NORMAL" RANGE WHILE THE LESS THE EXTENT OF CAD, THE MORE PRONOUNCED THE IMPROVEMENT. REEVALUATION OF THE STANDARD ST DEPRESSION CRITERION JOINTLY WITH THE ΔΜΑ VALUES ENHANCED THE SENSITIVITY OF THE METHOD FROM 71% TO 89%. B) THE "Y" GROUP SHOWED A SIMILAR ΔΜΑ BEHAVIOR TO THAT OF "Σ". C) THE ST SCORE ENHANCED THE SENSITIVITY OF THE METHOD, REFLECTING MORE ACCURATELY THE TOTAL ISCHEMIC RESPONSE DURING ETT. IN CONCLUSION THE ΔΜΑ SUCCESSIVELY ENCOMPASSES IN ONE NUMBER ALL THE "HEMODYNAMIC" DATA FROM THE ETT AND INCREASES THE SENSITIVITY OF THE METHOD.
