Τα παιδιά και ιδιαιτέρως οι έφηβοι συχνά εμφανίζουν συγκοπτικά επεισόδια. Υπολογίζεται ότι μέχρι το τέλος της εφηβείας 15% των παιδιών θα εμφανίσουν τουλάχιστον ένα συγκοπτικό επεισόδιο. Τα αίτια της συγκοπής στα παιδιά όπως και στους ενήλικες είναι πολλά και του αυτού τύπου. Στα παιδιά όμως τα αίτια των συγκοπτικών επεισοδίων διαφέρουν σε ποσόστωση και στην πλειονότητά τους είναι καλοήθη. Προβληματίζουν για την πρόγνωση τα συγκοπτικά επεισόδια που οφείλονται σε παθήσεις του κυκλοφορικού και του ΚΝΣ, που όμως από κοινού αντιπροσωπεύουν μικρό ποσοστό (περίπου 5-7%). Η επινόηση και εφαρμογή από τριακονταετίας της δοκιμασίας ανακλινόμενης τράπεζας στους ενήλικες πέτυχε σε υψηλό ποσοστό τη διάγνωση των επεισοδίων συγκοπής που μεσολαβούνται δια του αυτόνομου νευρικού συστήματος. Η δοκιμασία στα παιδιά άρχισε να εφαρμόζεται τη δεκαετία 1990 χωρίς όμως να έχει τυποποιηθεί η εκτέλεσή της μέχρι σήμερα, με αποτέλεσμα η ευαισθησία και η ειδικότητα της μεθόδου να ποικίλλουν ευρέως. Η μελέτη μας περιλαμβάνει 128 παιδιά (52 αγόρια 76 κορίτσια) ηλικίας 6 μέχρι 18 ετών, μέσης ηλικίας 11±2,8 έτη και διάμεσης ηλικίας 11,2 έτη. Τα παιδιά αυτά εμφάνισαν συγκοπή (102 παιδιά) ή προσυγκοπή (26 παιδιά). Εκτεταμένος εργαστηριακός έλεγχος απέκλεισε τα οργανικά αίτια. Από τα 128 παιδιά 21 είχαν ένα επεισόδιο συγκοπής ή προσυγκοπής και 107 2-30 επεισόδια συγκοπής ή προσυγκοπής (μέσος αριθμός επεισοδίων 3,8±4,3 ανά παιδί). Θετικό ιστορικό συγκοπής σε 1ου βαθμού συγγενείς αναφέρθηκε σε 72 από τα 128 παιδιά (56,2%). Πρόδρομες κλινικές εκδηλώσεις εμφάνισαν 107 από τα 128 παιδιά (83.6%) με συγκοπή ή προσυγκοπή. Πιο συχνές απ’ αυτές ήταν: ζάλη, ωχρότητα, θάμβος όρασης, κεφαλαλγία, εφίδρωση, κοιλιακά άλγη και ανησυχία. Στο τελευταίο συγκοπτικό ή προσυγκοπτικό επεισόδιο 105 (82%) παιδιά ήσαν όρθια, 18 (14%) κάθονταν, 3 (2.3%) ήσαν ξαπλωμένα και 2 (1,6%) ασκούνταν. Ως μάρτυρες χρησιμοποιήθηκαν 44 παιδιά (21 αγόρια και 23 κορίτσια) ηλικίας από 6 ετών έως 14 ετών. Η μέση ηλικία τους ήταν 10,6±2,5 έτη και η διάμεση ηλικία 11,2 έτη με ελεύθερο ατομικό και οικογενειακό ιστορικό για συγκοπή. Τα δείγματα ασθενών και μαρτύρων δεν είχαν στατιστικά σημαντική διαφορά, όσον αφορά το φύλο και την ηλικία, άρα ήταν συγκρίσιμα. Η δοκιμασία ανακλινόμενης τράπεζας βρέθηκε θετική σε 91 από τα 128 παιδιά της ομάδας των ασθενών (71,1%) και σε κανένα από τα 44 παιδιά της ομάδας των μαρτύρων. Η δοκιμασία θετικοποιήθηκε σε 32 παιδιά στο πρώτο στάδιο, σε 13 στο 2ο, σε 30 στο 3ο και σε 16 στο 4ο. Τα παιδιά της ομάδας των μαρτύρων συγκρίθηκαν μόνο με την ομάδα των ασθενών του πρώτου σταδίου, δεδομένου ότι στους μάρτυρες δε θεωρήθηκε ηθικά αποδεκτή η χορήγηση ισοπροτερενόλης. Η δοκιμασία θετικοποιήθηκε σε 32 παιδιά από την ομάδα των ασθενών (25%) και σε κανένα από την ομάδα των μαρτύρων. Ο μηχανισμός επέλευσης της συγκοπής και θετικοποίησης της δοκιμασίας στα παιδιά της μελέτης ήταν καρδιοαναστολή (ελάττωση μόνο των σφύξεων) σε 11, αγγειοκαταστολή (πτώση μόνο της αρτηριακής πίεσης) σε 36 και η μικτή (συνδυασμός των δύο) σε 44 παιδιά. Κατά την επέλευση του επεισοδίου η μέση μείωση του καρδιακού ρυθμού στα παιδιά με μόνο καρδιοαναστολή ήταν 54±22 σφύξεις ενώ η μέση τιμή πτώσης της συστολική αρτηριακής πίεσης στα παιδιά με μόνο αγγειοκαταστολή ήταν 33±7 mmHg. Η σύγκριση των 91 ασθενών με θετική τη δοκιμασία ανακλινόμενης τράπεζας, με τους 37 ασθενείς με αρνητική έδειξε ότι: οι ασθενείς με θετική δοκιμασία είχαν μεγαλύτερη ηλικία (p=0,001), μεγαλύτερο αριθμό προηγούμενων συγκοπτικών επεισοδίων (p=0,024), υψηλότερη πιθανότητα θετικού οικογενειακού ιστορικού συγκοπτικών επεισοδίων (p=0,025) και μεγαλύτερη εκδήλωση πρόδρομων κλινικών εκδηλώσεων (p=0,001). Η δεκάλεπτη δοκιμασία ορθοστασίας και η μέτρηση του R-R διαστήματος του ΗΚΓ πριν και μετά την πίεση των οφθαλμικών βολβών έδειξε ότι οι ασθενείς με θετική τη δοκιμασία ανακλινόμενης τράπεζας, έχουν βαθμό βαγοτονίας.

In the last three decades, the application in clinical practice of the Tilt Table Test (TTT) or Head Up Tilt Test (HUT) in adults succeeded in diagnosing syncope episodes that are mediated by the autonomic nervous system in a standardized way as described by official scientific bodies. The application of the test in children started in the 1990s but its performance has not been standardized yet. Furthermore, the sensitivity and specificity of this method in children reportedly varies widely. Our study included 128 children (52 boys) aged 6 to 18 years with a mean age 11±2,7 years and median age 11,4 years. These children had episodes of syncope (109 children) or pre-syncope (28 children). We also recruited 44 children (21 boys and 23 girls) aged 6 years to 14 years as controls. Their mean age was 10,6±2,5 years and median age 11.2 years and had a free history for syncope either personal or familial. Our study population (patients and controls) were not significantly different in terms of gender and age and were comparable for statistical purposes. Extensive laboratory testing ruled out organic causes in all patients. Of the 137 children, 30 had one episode of syncope or pre-syncope and 107 2-30 episodes syncope or pre-syncope (mean number of episodes 3,8±4,3 per child). A positive history of syncope in first degree relatives was reported in 77 of the 137 children (56.2%). Prodromal clinical manifestations were reported in 115 of the 137 children (83.9%) with syncope or pre-syncope. Most common prodromal manifestations were: dizziness, pallor, blurred vision, headache, sweating, abdominal pain and restlessness. Of the 109 children with syncope episode, 81 (74.3%) dropped on the floor and 34 of them (42%) were slightly injured. In the final episode of syncope or pre-syncope, 105 (82%) children were at a standing position, 18 (14%) at a sitting position, 3 (2.3%) were at a supine position and 2 (1,6%) were performing an exercise. The vast majority of patients had episodes while at the upright position (82%), which suggests that orthostatic intolerance syndrome might be the underline aetiology. However, in our study we have excluded cardiac and neurological causes that are the most important underline pathologies. In our study, we have shown that the Tilt Test is highly specific because none of the controls gave a positive response to the test. While only 25% of patients yielded a positive response in the first stage (without isoproterenol administration), sensitivity was sufficiently high during stages 2, 3 and 4 with isoproterenol, (for the patients only, due to ethical reasons) as we had a positive response in about 61% of the patients. We also showed that the more severe prodromal symptoms in a patient, the greater the likelihood of dysfunction of the autonomic nervous system. The same was true with the report of a positive family history. Similar positive correlation exists with Valsava manipulations (increased in patients with bradycardia or greater drop in BP from the horizontal to the upright position). The Tilt Table Test was positive in 91 of the 128 children in the patients group (71.1%) and in none of the 44 children in the control group. The test was positive in 32 children in the first stage, 13 in the second, 30 in the third and 16 in the fourth. Children of the control group were only compared with patients for the first stage of the test since it was not ethically acceptable to administer them isoproterenol. The test was positive in 32 children from the patients group (25%) and in none in the control group. The mechanism of appearance of syncope and the positivity of the test in the children of this study was mediated via cardio-inhibition (decreased only pulses) in 11, vaso-depression (only fall in blood pressure) in 36 and via a mixed mechanism (a combination of both) in 44 children. At the onset of the episode, the mean decrease in heart rate in children that demonstrated only cardio-inhibition was 54±22 beats, while the average fall of the systolic blood pressure in children with only vaso-depression was 33±7 mmHg. A comparison of 91 patients with positive Tilt Table Test, with 37 patients with negative response showed that the patients with a positive response were older in age (p=0,001), had greater number of previous syncope episodes (p=0,024), reported more frequently a positive family history of syncope (p=0,025) and experienced more prodromal clinical manifestations (p=0,001). The blood pressure measurement after ten minutes standing and the measurement of the R-R interval on the ECG before and after Valsalva manoeuvre, showed that patients with positive tilt table test have at some extent vaso vagal trend.
