Είναι γνωστή η συμμετοχή των LDL λιποπρωτεινων στη δημιουργία αθηρωματικής πλάκας και θεωρούνται από τους σημαντικότερους παράγοντες στην ανάπτυξη στεφανιαίας νόσου. Η βλαπτική τους δράση οφείλεται αφ΄ενός στο μικρό της μέγεθος-εύκολη είσοδος στο ενδοθήλιο των αγγείων-και αφ΄ετέρου στην εύκολη οξείδωση των. Οι ως άνω μεταβολές σηματοδοτούν την έναρξη των διεργασιών της αθηρογένεσης. Οι HDL λιποπρωτεινες και κυριως οι υποομάδες της HDL2 και HDL3, ασκούν προστατευτική δράση και αποτρέπουν την δημιουργία αθηρωματικών πλακών και κατ΄επέκταση την ανάπτυξη στεφανιαίας νόσου. Ειδικότερα οι HDL λιποπρωτεινες αναστέλουν την οξείδωση των LDL λιποπρωτεινων με συνέπεια την άρση της βλαπτικής των δράσης στο ενδοθήλιο των αγγείων. Η δράση αυτή των ΗDL λιποπρωτεινων και των υποομάδων αυτής HDL2 και HDL3 ασκείται κυρίως μέσω ενός ενζύμου της παραοξονάσης. Η σημαντικότητα του ενζύμου αυτού έχει προκαλέσει το ενδιαφέρον πολλών ερευνητικών ομάδων διεθνώς. Επίσης είναι γνωστό ότι η ομοκυστεινη αποτελεί ανεξάρτητο παράγοντα κινδύνου δημιουργίας στεφανιαίας νόσου μέσω πολύπλοκων μηχανισμών, κυρίως μέσω τοξικής δράσης στο αγγειακό τοίχωμα με συνέπεια τη δυσλειτουργία του ενδοθηλίου πυροδοτώντας έτσι τους μηχανισμούς αθηρογένεσης. Σκοπός της διαδακτορικής διατριβής μας ήταν να καταγράψουμε σε Έλληνες πάσχοντες από στεφανιαία νόσο απεικονιστικά τεκμηριωμένη τις τυχόν λιπιδιακές διαταραχές κυρίως HDL,HDL2,HDL3 και τις μεταβολές της παραοξονάσης και ομοκυστεινης . Τα αποτελέσματα μας συμφωνούν εν γένει με εκείνα της διεθνούς βιβλιογραφίας και είναι τα κάτωθι:-Οι HDL,HDL2,HDL3 είναι πολύ χαμηλότερες σε ασθενείς με οξύ έμφραγμα από τους υγιείς, όχι όμως και στους πάσχοντες από στεφανιαία νόσο, πιθανόν λόγω λήψης στατινών.-Η βαρύτητα – αριθμός προσβεβλημένων αγγείων σε ασθενείς με χρόνια στφανιαία νόσο σχετίζονται αρνητικά με τις τιμές HDL2.-Διεπιστώθει επίσης σημαντική αρνητική συσχέτιση του αριθμού προσβεβλήμενων αγγείων με τις τιμές της παραοξονάσης.Τέλος η έκταση της αγγειακής βλάβης-αριθμός προσβεβλημένων αγγείων- σχετίζεται θετικά με τις τιμές της ομοκυστεινής. Από τα αποτελέσματα της παρούσης διδακτορικής διατριβής οδηγούμεθα στο συμπέρασμα ότι η αξιολόγηση των μελετηθέντων παραμέτρων συμβάλλουν σημαντικά στην διάγνωση, πρόγνωση, στο καθορισμό της βαρύτητας και στην εξέλιξη της στεφανιαίας νόσου.

It is also known that homocysteine acts as an independent risk factor for the development of coronary disease through intricate mechanisms, mainly through the toxic action in the vascular wall, resulting in the impairment of the endothelium, triggering this way the atherogenesis mechanisms. The aim of this doctoral thesis is to register the probable lipid disorders, especially HDL,HDL2,HDL3, and the changes in paraoxonase and homocysteine in those suffering from documented imaging coronary disease. Our results agree in general with those of the international bibliography and are the following:-The levels of HDL, HDL2, HDL3, are much lower in patients with an acute myocardial infarction than the healthy ones, however that is not the case in those suffering from coronary artery disease, probably because they take statins.- The severity-number of infected vessels in patients with chronic coronary disease are negatively related with the levels of HDL2.- We also found a significant negative correlation between the number of infected vessels and the levels of paraoxonase-Finally, the extent of the vascular damage - number of infected vessels - is positively related to the levels of homocysteine. From the findings of the present doctoral thesis, we are led to the conclusion that the evaluation of the studied parameters contribute significantly to the diagnosis, prognosis and determination of the severity and the progression of coronary disease.
